摘要
文章通过对《伤寒论》中火热证治规律进行梳理,认为火热证在《伤寒论》中辨治系统、特色鲜明。《伤寒论》火热理论发源于《黄帝内经》《难经》,火热病机来源分外感、内伤两方面,六经皆有热证,病位涉及五脏六腑。张仲景根据火热邪气六经、表里、虚实、阴阳之不同,将实火证、虚火证、虚实夹杂证分别论治,针对夹湿、夹痰、夹水、夹瘀治疗亦非常全面。治疗中重视因势利导,形成了以清热、宣透、攻下、养阴为主的辨治体系。其中渗透的"保胃气""存津液"理念,对后世医家尤其是明清温病学家影响深远,值得进一步深入探讨与研究。
The article sorted out the law of fire-heat syndrome in Shanghan Lun, and considered that the fire-heat syndrome is distinctive in the treatment system of Shanghan Lun. The theory of fire-heat in Shanghan Lun originated from Huangdi Neijing and Nanjing. The source of fire-heat pathogenesis was divided into two aspects: exogenous disease and internal damage. The six channels all had heat syndrome, and the disease locations involved the five zang viscera and six fu viscera. According to the differences of fire-heat pathogenic qi among the six channels, superficies and interior, deficiency and excess, and the yin and yang, ZHANG Zhong-jing treated the excess fire syndrome, the deficiency fire syndrome, and syndrome of intermingled deficiency and excess respectively. The treatment for diseases with damp, phlegm, water and stasis was also very comprehensive. In the treatment, attention was paid to the situation, and a system of differentiation and treatment based on clearing heat, releasing, purgation and nourishing yin was formed. The infiltrated concept of 'protecting the stomach qi' and 'preserving the fluid', which had far-reaching influence to future doctors, especially the Ming and Qing dynasties, deserves further exploration and research.
