摘要
目的探讨炎症性肠病患者感知病耻感现状及其对患者生活质量、服药依从性和心理状态的影响。方法采用方便抽样,选取南京市4所三级甲等医院住院或门诊的175例炎症性肠病患者,使用炎症性肠病感知病耻感量表、炎症性肠病生活质量量表、服药依从性量表、炎症性肠病自我效能量表、医院用焦虑抑郁量表及自制患者一般情况调查表对患者进行调查。结果炎症性肠病患者感知病耻感得分为(76.07±38.72)分,其中43.5%的患者存在中、重度感知病耻感。感知病耻感总分与生活质量、服药依从性、自我效能得分呈负相关(r=-0.508～-0.343,P<0.01),与焦虑抑郁得分呈正相关(r=0.514～0.558,P<0.01)。分层回归分析结果显示,感知病耻感独立解释患者生活质量变异的6.8%～21.0%,对服药依从性变异的解释为7.2%,分别占患者焦虑抑郁变异的26.4%和31.2%,以及自我效能变异的12.8%～17.0%。结论炎症性肠患者普遍存在感知病耻感,在控制人口学、疾病资料后,感知病耻感可降低患者生活质量、服药依从性和自我效能感,加重患者焦虑、抑郁情绪,应引起医护工作者的关注。
Objective To investigate the perceived stigma in patients with inflammatory bowel disease and its influence on psychology,medication compliance and quality of life. Methods The convenience sampling was used to select 175 patients with inflammatory bowel disease who were hospitalized or outpatients in four hospitals in Nanjing. Measures included the Perceived Stigma Scale in IBD （PSS-IBD）,Inflammatory Bowel Disease Question- naire,Morisky Medication Adherence Scale,Inflammatory Bowel Disease-Self-Efficacy Scale and Self-Designed Gener- al Information Questionnaire. Results The seore of the perceived stigma in patients with inflammatory bowel dis- ease was （76.07±38.72）,and 43.5% of the patients had moderate to severe perceived stigma. The total score of PSS-IBD and its social dimensions were all negatively correlated with patients＇ quality of life,medication compliance and self-efficacy（r=-O.508～-O.343,P〈O.O1）,and positively correlated with anxiety and depression（r=O.514～O.558,P〈 0.01）. Hierarchical regression demonstrated that perceived stigma accounted for 6.8～21.0% of the variance in scores for quality of life,7.2% for medication adherence,26.4% ,31.2% for anxiety and depression,and 12.8～17.0% for self-efficacy. Conclusion The perceived stigma is prevalent in patients with inflammatory bowel disease. Perceived stigma may increase anxiety,depression,impair self-efficacy,medication adherence and quality of life when control- ling for demographic and clinical variables,to which attention should be paid.
