摘要
目的分析基底节区高血压脑出血CT分级与预后的相关性。方法选取2015年3月至2017年3月本院收治的基底节区高血压脑出血患者96例。按照患者的环池以及脑干形态学变化进行CT分级,并应用Spearman相关性分析分析不同CT分级患者的预后情况、GCS评分以及脑出血量的相关性。结果随患者CT分级的不断升高,患者预后分级逐渐升高,脑出血量也呈逐渐上升趋势,而患者GCS评分逐渐降低,且两组间均存在显著性差异(P<0.05)。经Spearman相关性分析发现:基底节区高血压脑出血CT分级和预后以及脑出血量呈显著正相关关系(r=0.689,0.672;P<0.05),而与GCS评分呈显著负相关关系(r=-0.729;P<0.005)。结论基底节区高血压脑出血CT分级与患者的预后存在显著相关性,临床工作中可通过对患者及进行CT诊断,从而有效预测结局,对高血压脑出血的临床治疗具有极其重要的指导意义。
Objective To analyse the relationship of CT grading and prognosis on hypertensive intracerebral hemorrhage in basal ganglia. Methods From March 2015 to March 2017, 96 patients with hypertensive intracerebral hemorrhage in basal ganglia were selected. CT grading was performed according to the morphological changes of the cistern and brainstem of the patient, and Spearman correlation analysis was used to analyze the prognostic status of patients with different CT grades and the correlation between GCS score and cerebral hemorrhage volume. Results With the increasing of CT grade, the prognosis grade and the amount of cerebral hemorrhage increased gradually, while the GCS score of the patients decreased gradually, and there was significant difference between the two groups ( P 〈0.05). By spearman correlation analysis, it was found that there was a significant positive correlation between CT grade, prognosis and cerebral hemorrhage volume in hypertensive intracerebral hemorrhage in basal ganglia( r =0.689,0.672; P 〈0.05), but there was a significant negative correlation with GCS score and CT grade ( r =-0.729, P 〈0.05). Conclusion There have the significant correlation between CT grade and prognosis of hypertensive intracerebral hemorrhage in basal ganglia. In clinical work, CT diagnosis can be used to predict the outcome of hypertensive intracerebral hemorrhage, which is of great significance to the clinical treatment of hypertensive intracerebral hemorrhage.
