摘要
目的:分析微创颅内血肿清除术在高血压脑出血患者治疗中应用效果,为高血压脑出血治疗方案选择提供一定依据。方法:选取2015年6月~2017年6月某院收治84例高血压脑出血患者作为研究对象,按照随机数字表将其分为常规治疗组(n=42)及微创治疗组(n=42),常规治疗组患者采用传统颅内血肿清除术,微创治疗组采用微创颅内血肿清除术,对比两组患者治疗后临床效果、神经功能缺损评分、出血量、血肿周围水肿量、炎性因子水平、SP、SF变化及并发症发生情况。结果:接受治疗之后,微创治疗组的总有效率为92.86%,常规治疗组的总有效率为78.57%(P<0.05);微创治疗组的并发症发生率为9.52%,常规治疗组的并发症发生率为21.43%(P<0.05);微创治疗组的神经功能缺损评分、出血量及血肿周围水肿量、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)及高敏C反应蛋白(hs-CRP)水平、SF明显低于常规治疗组(P<0.05);微创治疗组的SP水平明显高于常规治疗组(P<0.05)。结论:在高血压脑出血治疗方面,采用微创颅内血肿清除术能够明显提高其临床疗效,减少并发症的发生,安全性高,能有效促进患者神经功能恢复,明显改善炎症因子与SP、SF水平,值得推广。
Objective: To analyze the application effect of minimally invasive intracranial hematoma removal in the treatment of patients with hypertensive cerebral hemorrhage,and provide some basis for the selection of treatment options for hypertensive cerebral hemorrhage. Methods: 84 cases of hypertensive cerebral hemorrhage patients treated in a hospital from June 2015 to June 2017 were divided into conventional treat ment group ( n =42) and the minimally invasive treatment group ( n =42) according to the random Numbers table.Conventional treatment group were treated by traditional removal of intracranial hematoma, minimally invasive treatment group used minimally invasive removal of intracranial hematoma.Compared posttreatment clinical effect, neural function defect scale, amount of bleeding, hematoma surrounding edema and inflammatory factor levels, SP, SF, change, and complications of two groups. Results: After treatment, the total effective rate of minimally invasive treatment group was 92.86%, and the total effective rate of routine treatment group was 78.57% ( P 〈0.05). The complication rate of minimally invasive treatment group was 9.52% , and the complication rate of routine treatment group was 21.43% ( P 〈0.05). Neural function defect scale, the amount of bleeding and edema around the hematoma volume, tumor necrosis factor-α (TNF-α), interleukin 6 (IL-6) and high-sensitivity c-reactive protein (hs-CRP) level, SF of minimally invasive treatment group were significantly lower than that of the routine treatment group ( P 〈0.05); The SP level of minimally invasive treatment group was significantly higher than that of conventional treatment group ( P 〈0.05). Conclusion: In treatment of hypertension cerebral hemorrhage, the use of minimally invasive removal of intracranial hematoma can significantly improve the clinical curative effect, reducing the occurrence of complications, high safety, effectively promoting patients with nerve functional recovery, significantly improving the inflammatory factor and SP or SF levels,it is worth promoting.
