血液学指标在细菌感染性疾病辅助诊断中的价值分析 被引量:11

Auxiliary diagnostic value of hematological indices in bacterial infections

摘要 目的探讨外周白细胞计数(WBC)、中性粒细胞百分比(N%)、C反应蛋白(CRP)和降钙素原(PCT)检测对细菌感染性疾病辅助诊断的价值。方法收集2016年1月至2017年10月,略阳县人民医院确诊为细菌感染的患者90例为试验组,选择同期确诊为非细菌感染疾病患者90例为对照组。两组患者均予测定WBC、N%、CRP和PCT指标,比较分析两组患者指标检测结果及阳性率。结果试验组患者WBC(t=2.305,P=0.024);N%(t=5.214,P=0.009);CRP(t=20.047,P=0.000)和PCT(t=19.501,P=0.000)均高于对照组,差异有统计学意义。试验组患者WBC、N%、CRP和PCT的阳性率均高于对照组,差异有统计学意义(χ~2=24.112、70.005、49.630、88.028,均P=0.000)。试验组患者N%、CRP和PCT的阳性率均高于WBC,差异有统计学意义(χ~2=23.243、46.283、28.093,均P=0.000);CRP阳性率高于PCT,差异有统计学意义(χ~2=8.004,P=0.031)。对照组患者CRP阳性率均高于WBC、N%和PCT,差异有统计学意义(χ~2=18.362、19.824、22.018,均P=0.000)。结论WBC、N%、CRP和PCT指标能够辅助细菌感染性疾病的诊断,N%、CRP和PCT对细菌感染的诊断要优于WBC,CRP的敏感性最高,但特异性较低,而PCT敏感性和特异性均高,更适用于细菌感染性疾病的诊断。 Objective To discuss the use of hematological indices as peripheral white blood cell count(WBC),percentage of neutrophils(N %),C-reactive protein(CRP)and calcitonin(PCT)in the diagnosis of bacterial infections. Methods 90 patients with confirmed bacterial infections and 90 patients with non-bacterial infections as controls in The People′s Hospital of Lueyang from January 2016 to October 2017 were enrolled into the study.Blood samples were collected and WBC,N %,CRP and PCT were measured and compared. Results The WBC count(t=2.305,P=0.024),N%(t=5.214,P=0.009),CRP(t=20.047,P=0.000)and PCT(t=19.501,P=0.000)of patients with bacterial infections were all higher than that of controls,the differences were statistically significant.The positive rates of WBC,N %,CRP,and PCT were higher among patients with bacterial infections compared to controls,and the differences were statistically significant(χ^2=24.112,70.005,49.630,88.028,all P=0.000).The positive rates of N %,CRP,and PCT were all higher than that of WBC among patients with bacterial infections,and the differences were statistically significant(χ^2=23.243,46.283,28.093,all P=0.000),and the positive rate of CRP was significantly higher than that of PCT(χ^2=8.004,P=0.031).The positive rate of CRP in controls was higher than that of WBC,N%,and PCT,the differences were statistically significant(χ^2=18.362,19.824,22.018,all P=0.000). Conclusions WBC,N%,CRP,and PCT can be used as an auxiliary diagnostic potentials for bacterial infections,especially the last three indices.CRP is more sensitive but less specific,while PCT is both sensitive and specific which is more suitable for the diagnosis of bacterial infections.
