摘要
目的 :分析动机性访谈(MI)在前交叉韧带重建术后患者功能训练中应用的效果。方法 :采用便利抽样法,选取2016年3月至2017年10月某三级甲等医院同一组医生收治的单纯前交叉韧带损伤关节镜下行前叉韧带重建术患者106名,随机分为对照组和干预组,对照组实施常规的前叉韧带重建术功能锻炼干预,干预组应用MI实施干预,两组均按规定时间进行4次干预,于术后3天、1周及1个月对两组患者的一般自我效能感、功能锻炼依从性、膝关节Lysholm评分进行评价。结果 :术后3天患者的自我效能感、膝关节功能评分及功能训练依从性干预组和对照组组间比较无统计学差异(P>0.05)。术后1周患者的自我效能感、功能训练依从性干预组优于对照组(P<0.05),差异有统计学意义;膝关节Lysholm评分干预组和对照组组间比较无统计学差异(P>0.05)。术后1个月患者的自我效能感、膝关节功能评分及功能训练依从性干预组优于对照组(P<0.05),差异有统计学意义。结论 :对前交叉韧带重建膝关节术后患者基于MI进行功能锻炼的干预,可以提高患者的自我效能感和功能锻炼的依从性,功能锻炼干预效果较传统干预方法具有显著的优势。
Objective： To evaluate the effect of Motivational Interviewing on the functional training in postoperative patients with the anterior cruciate ligament reconstruction. Methods： Totally 106 patients with the anterior cruciate ligament reconstruction surgery, who were treated by the same doctor, were divided into two groups. The patients in the control group received the routine functional training education intervention of the anterior cruciate ligament reconstruction surgery; while patients in the experimental group received 4 times motivational Interviewing intervention. GSES, the knee evaluation scale of Lysholm and the functional training compliance were measured before and after intervention at three moments： three days, a week, and one month after the surgery. Results： There were no significant different between two groups in the scores of GSES, the Lysholm score of knee and the functional training compliance after the surgery at 3 days （P〉0.05）. The GSES and the functional training compliance of the experimental group was improved significantly than that of control group after the surgery at 7 days （P〈0.05）. The GSES, the Lysholm score of knee and the functional training compliance of the patients of experimental group were significantly improved than those of control group after the surgery at 1 month （P〈0.05）. Conclusion： Compared to routine education,the motivational Interviewing may improve the GSES, the Lysholm score of knee and the functional training compliance in postoperative patients with the anterior cruciate ligament reconstruction more effectively.
