摘要 脉诊是中医基本诊断技术,对脉诊的研究是中医学领域的重要研究方向。从传统中医角度来看,把握阴阳气血以及人体内气化过程的规律是研习脉学的重要途径。中西医结合过程中,将干细胞分裂分化过程和能量代谢作为人体生命现象中两个核心的生理过程,并且作为阴阳概念的生理学基础。在以血管为核心的局部组织器官中,能量代谢过程和干细胞分裂分化过程仍然是组织器官生理代谢和细胞更新的核心所在,并构建了血管、血管内容物以及血管外组织之间的相互关系,即血管内的能量物质被氧化成能量的代谢终产物,同时血管内的干细胞迁徙到细胞血管外分裂分化以替代死亡细胞。因此,对脉象的研究就不能局限于桡动脉波形、节律,更需要关注桡动脉三维空间、血管壁、血管内容物以及血管同周围组织的关系。这样不仅能够掌握临床常见脉象,而且还能够通过上述脉象揣度人体内气血运行状态从而形成对病机的把握。 Pulse manifestation is an essential method for diagnosis in Traditional Chinese Medicine (TCM). Researches on pulse manifestation are also an important field for modernization of TCM. In TCM, the understandings on the concepts of yin, yang, qi, xue and the motion of qi are the ways for studying and utilization of pulse manifestation. In the integration of TCM and modern medicine, energy metabolism and stem cell division & differentiation are the essential pathways in our body, which should be regarded as yang and yin respectively. Furthermore, energy metabolism and stem cell division & differentiation also play the dominant role and become the bridge connecting the vascular content, blood vessel and extravascular tissue: the energy agents in blood vessel will be oxidized into end metabolic products while the stem cell will migrate from blood vessel to local tissue and produce the somatic cells to take the place of dead ones. So, researches on pulse manifestation should focus on following factors, such as the three dimensions of radial artery, arterial wall, the contents in radial artery as well as relationships between the radial artery and local tissues beyond the waveform and rhythm of radial pulse. In this way, clinicians can not only feel the normal pulse manifestations but also surmise the states of qi and xue in patient so as to make the accurate diagnosis in TCM.
