摘要
目的探究多层螺旋CT多平面重组和容积再现三维成像对四肢骨关节骨折的诊断价值。方法前瞻性研究,选择2015年12月至2017年11月喀什地区第二人民医院收治的60例四肢骨关节骨折的患者作为研究对象,对所有患者的骨折部位进行X线检查和多层螺旋CT影像检查,并进行多层螺旋CT多平面重组三维成像和容积再现三维成像。比较不同影像学检查方法诊断四肢骨关节骨折的检出率。结果 88处四肢骨关节骨折中,X线检查显示,55处关节面断裂,21处关节面塌陷,18处关节内碎骨片,8处关节脱位;多平面重组三维成像显示,82处关节面断裂,36处关节塌陷,30处关节内碎骨片,14处关节脱位;容积再现三维成像显示,80处关节面断裂,34处关节塌陷,30处关节内碎骨片,15处关节脱位。多层螺旋CT扫描后经多平面重组三维成像以及容积再现三维成像的检出率均显著性大于X线检查的检出率(P<0.05)。结论多层螺旋CT多平面重组和容积再现三维成像对四肢骨关节骨折的对骨折面诊断价值较X线检查更高。
Objective To explore the diagnostic value of multiplanar reformation and volume rendering 3D imaging of multislice spiral CT for extremities bone and joint fractures. Methods This was a prospective study. From December 2015 to November 2017, 60 cases of limbs bone and joint fractures accepted in Second People's Hospital of Kashi were selected as the research subjects. X-ray films and multi-slice spiral CT were used to examine the fracture sites. Multi slice spiral CT multiplanar reconstruction 3D imaging and volume rendering 3D imaging were performed. To compare the diagnostic rate of different imaging methods in diagnosing bone and joint fractures of limbs. Results The X-ray results showed 55 fracture of articular surface, 21 joint surface collapse, 18 intra-articular fragments and 8 joint dislocation, and 82 joint surface fracture, 36 joint collapse, 30 intra-articular fragment, 14 joint dislocation, and 80 joint surface fracture and 34 joint collapse. 30 joints were broken in the joints and 15 joints were dislocated. After multislice spiral CT scan, the detection rate of multiplanar reconstruction three dimensional imaging and volume rendering 3D imaging is significantly greater than that of X-ray film ( P 〈0.05). Conclusion Multi slice spiral CT multiplanar reconstruction and volume rendering 3D imaging are of higher value in the diagnosis of fracture of the extremities and joints.
