摘要
目的探讨苦柏颗粒冷敷对面部激素依赖性皮炎患者IL-4、IgE及皮肤屏障功能的影响。方法将126例激素依赖性皮炎患者随机分为中药组66例,西药组60例。中药组给予苦柏颗粒外敷,对照组给予他克莫司乳膏,每日一次外涂,治疗4周后对比治疗前后患者IL-4、IgE及皮肤屏障功能的变化。结果尽管2组患者有效率大致相同,但中药组治愈率显著高于西药组,差异具有统计学意义(P<0.05);2组患者皮肤屏障功能较治疗前均有显著改善(P<0.05),但中药组改善较西药组更为明显,差异具有统计学意义(P<0.05);2组患者IL-4及IgE水平较治疗前均显著降低(P<0.05),但中药组降低更为明显,差异具有统计学意义(P<0.05);中药组患者复发率显著低于西药组,差异具有统计意义(P<0.05);2组患者在安全性评估方法无统计学差异(P>0.05)。结论苦柏颗粒冷敷可以显著改善激素依赖性皮炎患者皮肤屏障功能,其中机制可能与降低IL-4、IgE水平有关,同时其临床痊愈率高、复发率低、安全性良好,值得在临床进一步推广。
Objective To investigate the effect of cold compress with Kubo granules on IL-4,IgE and skin barrier function in patients with facial hormone-dependent dermatitis. Methods 126 cases of patients with hormone-dependent dermatitis were randomly divided intotraditional Chinese medicine（ TCM） group of 66 cases and western medicine group of 60 cases. Patients in the TCM group were treated with Kubo granule through external application,while the patients in the control group were treated with Tacrolimus cream（ qd,external application）. After 4 weeks of treatment,the changes of IL-4,IgE and skin barrier function in patients before and after treatment were compared. Results Although the efficiencies in the two groups were similar,the cure rate in the TCM group was significantly higher than that in the western medicine group,and the difference was statistically significant（ P 0. 05）. The skin barrier functions in both groups were significantly improved after treatment（ P 0. 05）. However,the improvement in the TCM group was more obvious than that in the western medicine group,and there had statistical significance（ P 0. 05）. The levels of IL-4 and IgE in the patients of both groups after treatment were significantly lower than those before treatment（ P 0. 05）,while in the TCM group,the levels decreased more obviously,and the difference had statistical significance（ P 0. 05）. The recurrence rate in the TCM group was significantly lower than that in the western medicine group,and the difference was statistically significant（ P 0. 05）. In the aspect of safety assessment,no statistical difference was found between the two groups. Conclusion Cold compress with Kubo granules can significantly improve the skin barrier function in patients with hormone-dependent dermatitis,for which the mechanism may be related to the reduction of IL-4 and IgE levels. Meanwhile,since it has high clinical cure rate,low recurrence rate and good safety,this therapeutic method is worth further clinical promotions.
