摘要
目的对糖尿病非酮症性高渗状态致偏侧舞蹈症(HC-NH)临床特征进行探讨。方法对该院2015年3月—2017年3月期间收治的9例糖尿病非酮症性HC-NH患者临床资料进行回顾性分析,对患者症状、发病机制、影像学资料及治疗转归情况进行总结。结果对糖尿病性非酮症性HC-NH患者进行影像学检查,CT表现为基底节稍高密度,MRI显示T1信号短,T2信号稍短或不变,周围无水肿,边界清晰;治疗后,患者血糖、渗透压、血清尿素氮水平分别为(11.3±0.67)mmol/L、(11.4±3.9)mmol/L、(284.7±5.9)m Osm/L,与治疗前对比,均明显下降,前后对比差异有统计学意义(t=5.372、5.643、7.183,P<0.05)。结论糖尿病非酮症性高渗状态致HC-NH疾病,主要特点表现为一侧脑组织影像学改变、偏侧舞蹈症及非酮症高血糖,老年女性群体中HC-NH多发,发病急,血糖升高明显,尿酮体呈弱阳性或阴性,对患者血糖进行有效控制,短期内症状即可缓解。
Objective To investigate the clinical features of hemiplegic chorea (HC-NH) caused by non-ketotic hyperosmolar state in diabetes. Methods The clinical data of 9 patients with non-ketotic HC-NH who were admitted to the hospital from March 2015 to March 2017 were retrospectively analyzed. The symptoms, pathogenesis, imaging data and treatment outcomes were analyzed. Results The imaging examination was performed on diabetic non-ketotic HC-NH patients. CT showed a slightly higher density of basal ganglia. MRI showed short T1 signal, T2 signal was short or unchanged, no edema around, and the boundary was clear. After treatment, The blood glucose, osmotic pressure and serum urea nitrogen levels were (11.3±0.67) lnlnol/L, (11.4±3.9) mmol/L, (284.7±5.9) lnOsln/L, respectively, which were significantly decreased coinpared with those before treatment. The difference was statistically significant (t=5.372, 5.643, 7.183, P〈0.05). Conclusion HC-NH disease caused by non-ketotic hyperosmolar state of diabetes is characterized by imaging changes of one side brain tissue, hemiplegic chorea and non-ketotic hyperglycemia. HC-NH is frequently found in elderly women, and the incidence is acute. The blood glucose rises obviously, and the ketone body is weakly positive or negative, which effectively controls the blood glucose of the patient, and the symptoms can be alleviated in a short period of time.
