摘要
目的探讨静脉注射免疫球蛋白对新生儿溶血免疫功能及临床效果的影响。方法将本院收治的新生儿溶血性黄疸患儿98例随机分为两组,各49例。对照组患儿采用双面蓝光间歇光疗联合苯巴比妥嗅化钠治疗,观察组在对照组基础上联合使用丙种球蛋白。比较两组患儿的临床效果。结果治疗后72 h,观察组患儿血清IgA、IgG和IgM水平均明显高于对照组(P<0.05),观察组CD4^+、CD8^+水平高于对照组(P<0.05),观察组CD4^+/CD8^+基本正常且高于对照组(P<0.05);治疗后24、48、72 h,观察组血清胆红素水平均显著低于对照组(P<0.05);观察组光照时间、住院时间均短于对照组(P<0.05)。结论丙种球蛋白治疗新生儿溶血效果显著,能有效改善机体免疫能力,降低黄疸水平,值得临床推广。
Objective To investigate the effect of intravenous immunoglobulin on hemolytic immune function and clinicalefficacy of newborns. Methods A total of 98 neonates with hemolytic jaundice treated in our hospital were randomly dividedinto two groups, with 49 cases in each group. The control group received double-sided blue light intermittent phototherapycombined with phenobarbital olfactory sodium, and the observation group used gamma globulin on the basis of the controlgroup. The clinical effects of the two groups were compared. Results At 72h after treatment, the levels of serum IgA, IgGand IgM in the observation group were significantly higher than those in the control group(P〈0.05), the levels of CD4+andCD8+in the observation group were significantly higher than those in the control group (P〈0.05), and the CD4+/CD8+werebasically normal and higher than that in the control group(P〈0.05). At 24, 48 and 72 h after treatment, the levels of serumtotal bilirubin in the observation group were significantly lower than those in the control group (P〈0.05). The illuminationtime and hospitalization time in the observation group were shorter than those in the control group (P〈0.05). ConclusionGamma globulin in the treatment of neonatal hemolysis is significantly effective, which can improve the bodys' immunecapacity and reduce the level of jaundice. It is worthy of clinical promotion.
