摘要
目的通过收集不同胎龄(GA)早产儿随访的Gesell发育量表评价结果,了解其校正年龄6个月时神经发育情况,以期尽早干预,改善预后。方法收集2016年4月至2017年3月于陆军军医大学第二附属医院就诊的81例早产儿资料和校正6月龄时Gesell发育量表测定的发育商(DQ),将其按GA分为<32周组、32~34+6周组、35~36+6周组,并对数据进行统计分析。结果 GA<32周组早产儿在粗大运动、精细运动、适应性行为、个人-社会性行为四个能区DQ<75分人数显著多于GA为32~34+6周组、35~36+6周组早产儿,经比较差异均有统计学意义(χ2值分别为9.989、63871、6.543、7.918,均P<0.05),语言能区差异无统计学意义(χ2=5.456,P=0.065);另外,GA<32周组早产儿五个能区评分显著低于GA为32~34+6周组、35~36+6周组的早产儿,差异具有显著统计学意义(F值分别为14.704、14.775、14.012、15.315、12.424,均P<0.001)。结论 GA<32周早产儿是发生神经系统发育不良的高危人群,需积极预防处理,密切随访。
Objective To investigate the neurological development of preterm infants of different gestational age(GA)at correct age of 6 months by collecting the results of Gesell Development Scale to give them effective intervention as early as possible and improve prognosis.Methods Clinical data of 81 premature infants visiting Second Affiliated Hospital of Army Medical University from April 2016 to March 2017 and Development Quotient(DQ)tested in Gesell Development Scale at correct age of 6 months were collected,and the cases were divided into group with GA32 weeks,group with GA of 32-34+6 weeks,and group with GA of 35-36+6 weeks for data analysis.Results Compared to group with GA of 32-34+6 weeks and group with GA of 35-36+6 weeks,there were more infants with QD75 in four function areas of gross motor movement,fine movement,adaptability and personal-social contact in group with GA32 weeks,and the differences were significant(χ^2 value was 9.989,63871,6.543 and7.918,respectively,all P〈0.05),but the difference in language function was not statistically significant(χ^2=5.456,P=0.065).In addition,the DQ of five function areas of infants in group with GA32 weeks was remarkably lower compared to that in group with GA of 35-36+6 weeks or 32-34+6 weeks with significance(F value was 14.704,14.775,14.012,15.315 and 12.424,respectively,all P 0.001).Conclusion The premature infants with GA32 weeks are group with high risk of neurological abnormality,so prompt intervention and close follow-up are needed.
