System B工作时间及次数对前磨牙牙根表面温度影响研究

Study on the effect of System B working time and times on the temperature of premolar root surface

摘要 目的研究System B多次加热后牙根表面温度变化及不同设置温度、工作时间对牙根表面温度的影响。方法选取2017年1—6月在南京大学医学院附属口腔医院口腔颌面外科就诊患者拔除的前磨牙60颗,采用System B常规操作步骤进行根管充填,对一次性不能取出中上段多余牙胶的前磨牙,进行反复3次加热操作。从放入System B热压尖开始,即将热电偶与距根尖5 mm处牙根近中面相接触,测量牙根表面温度变化,并检测调整System B设置温度和工作时间对牙根表面温度的影响。结果 21颗(35%)前磨牙中上段多余牙胶不能被一次性取出。System B反复3次加热4 s、冷却10 s后,牙根表面温度持续上升,第2次加热后牙根表面温度差普遍超过10℃;降低设置温度、延长冷却时间,第2次加热后牙根表面温度差仍会超过10℃;缩短加热时间,可控制牙根表面温度差在10℃以下。结论 System B加压充填时,特别是对薄根管壁及一次加热不能取出上段牙胶的牙齿,要缩短加热时间,尽量控制在2 s以内,以免对牙周组织造成损伤。 Objective To investigate the effect of System B on root surface temperature after repeated heating and the effect of different setting of temperature and working time on root surface temperature. Methods Totally 60 premolar teeth are collected and System B are used to obturate root canals. The teeth that could not be taken out of the upper Gutta Percha at one time are subjected to repeated heating operations for three times. The thermocouple is brought into contact with the mesial aspect of the roots at 5 mm from the root apex to measure the change of temperature,and measure the temperature after adjusting the setting of temperature and working time. Results Our result revealed that 35% of upper Gutta Percha of premolar teeth can not be removed once. When repeated three times,the root surface temperature continued to rise,the root surface temperature could rise more than 10℃ after the second heating. Reducing the set temperature or extending the cooling time could not significantly reduce the temperature;but by shortening the heating time,the root surface temperature could be controlled within 10℃ of increase. Conclusion When the System B device is used to obturate root canals,especially for the teeth with thin root canal wall and whose upper gutta-percha can not be removed at one time,the heating time should be shortened,being controled within 2 s,so as not to damage periodontal tissue.
