摘要
目的:探讨彩色多普勒超声在诊断锁骨下动脉窃血综合征(SSS)中的临床价值。方法:选取78例临床怀疑SSS的患者作为研究对象,对锁骨下动脉、颈动脉、椎动脉和上肢动脉行彩色多普勒超声,分析血流动力学,进行分型,并依据影像学特点判断病因,然后行DSA检查,比较两者结果。结果:(1)彩色多普勒超声诊断SSS共68例,其中部分型窃血28例,完全型窃血26例,隐匿型窃血14例;阴性10例。68例中病因为动脉粥样硬化60例,大动脉炎5例,夹层2例,锁骨下动脉发育不良1例。(2)DSA诊断SSS共71例,其中部分型窃血32例,完全型窃血24例,隐匿型窃血15例;阴性7例。病因为动脉粥样硬化63例,大动脉炎3例,夹层2例,不确定3例。与DSA比较,彩色多普勒超声对SSS的诊断灵敏度为92.96%,特异度为71.43%,准确率为91.03%,彩色多普勒超声能有效诊断SSS。结论:彩色多普勒超声对SSS具有重要的诊断价值,值得临床推广应用。
Objective:To investigate the diagnostic value of color Doppler ultrasound in subclavian steal syndrome (SSS).Methods:Based on a random sample of 78 patients with SSS by color Doppler ultrasonography,the color Doppler imaging of the carotid artery and the upper extremity of the subclavian artery was recorded and the hemodynamic was analyzed,to confirm the diagnosis of all patients who underwent digital subtraction angiography. Then DSA was adopted to compare the results. Results: ①Of the 78 patients,68 were diagnosed with subclavian steal by color Doppler ultrasonography,of which 28 were partial steal,26 complete steal,14 occult steal and 10 negative. In artery steal syndrome,60 cases were caused by atherosclerosis,6 by arteritis and 2 by dissection. ②Among the 78 patients,71 cases were diagnosed by DSA with subclavian steal,of which 32were partial steal,24 complete steal,15 occult steal and 7 negative. 63 patients were caused by atherosclerosis,3 by arteritis,2by dissection and 3 by uncertain reasons. Compared with DSA,ultrasound had a sensitivity of 92.96%,a specificity of 71.43%!and an accuracy of 91.03%. Ultrasound effectively diagnosed SSS. Conlusions:Color Doppler ultrasonography has important diagnostic value for patients with subclavian artery,even better than DSA,and is worthy of clinical application.
