摘要
目的探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者血清脂联素、瘦素、IL-8及8-异前列腺素F2α水平与肺功能的关系。方法选择2014年1月至2015年12月在新疆维吾尔自治区人民医院老年医学中心一病区的COPD患者60例为病例组以及同期于我院体检的健康志愿者为对照组,通过酶联免疫吸附法分别检测两组血清中脂联素、瘦素、IL-8及8-异前列腺素F2α的水平,并测定肺功能。结果 (1)病例组和健康对照组体重指数、胆固醇、甘油三脂、空腹血糖及收缩压和舒张压比较,差异均无统计学意义(P> 0. 05);(2)病例组血清脂联素、瘦素、IL-8及8-iso-PGF2α水平均高于健康对照组,差异有统计学意义(P <0. 05);(3)肺功能Ⅰ-Ⅳ级患者血清IL-8及8-iso-PGF2α水平逐渐上升,而瘦素、脂联素的水平却在逐渐下降,不同级别肺功能患者各项指标比较,差异均有统计学意义(P <0. 05);(4)Ⅰ-Ⅳ级肺功能患者的肺功能指标FEV1、FVC、FEV1/FVC水平均逐渐下降,差异均有统计学意义(P <0. 05);(5)病例组患者血清脂联素、瘦素、IL-8及8-iso-PGF2α与FEV1%、FVC%和FEV1/FVC肺功能指标相比均呈正相关(P <0. 05)。结论血清脂联素、瘦素、IL-8及8-iso-PGF2α的水平与COPD患者病情的发生、发展可能存在一定的联系,加强对这几项指标水平的监测和控制,将对COPD患者肺功能减退的控制有重要的临床意义,可以提高患者的临床治疗效果。
Objective To explore the relationship belween serum adiponectin, leptin, IL-8 and urinary 8 isoprostaglandin F2α and pulmonary function in patients with chronic obstructive pulmonary disease. Methods 60 patients with chronic obstructive pulmonary disease (COPD) from January 2014 to December 2015 in People's Hospital of Xinjiang Autonomous Region were selected as the study group, and 60 eases of healthy volunteers were taken as the control group. The serum levels of adiponeetin, leptin, IL-8 and 8-isoprostaglandin-F2α in the two groups were measured and lung function was detected. Results (1) There was no significant difference in body mass index, cholesterol, triglyceride glycerol, glucose, systolic and diastolic blood pressure between the two groups (P 〉 0. 05). (2) The levels of serum adiponectin, leptin, IL-8 and 8-iso-PGF2α in the observation group were higher than those in the healthy control group ( P 〈 0. 05 ). ( 3 ) The levels of serum IL-8 and 8-iso-PCF2α were gradually increased in patients with grade Ⅰ - Ⅳ puhnonary function, while the levels of leptin and adiponectin were gradually decreased. There were significant differences in the indexes of different grades of pulmonary function patients (P 〈 0. 05). (4) Their FEV1, FVC and FEV1/FVC were gradually decreased, and the differences were statistically significant (P 〈 0. 05). (5) The levels of serum adiponectin, leptin, IL-8 and urinary 8-iso-PGF2α were positively correlated with FEV1% , FVC% and FEVl/FVC (P 〈 0. 05). Conclusion The levels of serum adiponectin, leptin, IL-8 and 8- iso-PGF2α may be associated with the development and progression of COPD. The monitoring and control of serum levels of these markers functional decline of the control has important clinical significance, which can improve the clinical treatment of patients.
