摘要
探讨滤泡树突细胞肉瘤(follicular dendritic cell sarcoma,FDCS)的临床表现、病理特点、免疫组织化学、影像学表现、鉴别诊断、治疗及预后,旨在加深临床医生对FDCS的认识。本文报道石河子大学医学院第一附属医院1例发生于右颈部的FDCS患者的临床和随诊资料,并结合国内外文献对该病进行分析,明确其诊断及治疗方法。该例患者临床表现为右颈部缓慢生长的无痛性肿块,术后局部复发。镜检:淋巴结结构破坏,大量淋巴细胞背景中肿瘤细胞呈束状、席纹状、漩涡状或弥漫性排列;瘤细胞呈梭形卵圆形,胞质丰富,可见核仁。免疫组织化学:瘤细胞S-100(+),CD21(+),CD23(+)。FDCS是一类罕见且易误诊的恶性肿瘤。发病率低、易复发和转移,影像学诊断困难,应用合理的免疫组织化学标记结合病理组织化学是避免误诊的必要条件。
To explore the clinical manifestations, pathological features, immunohistochemical features, imaging findings, differential diagnosis treatment and prognosis of follicular dendritic cell sarcoma（FDCS）, and aims to deepen the knowledge of follicular dendritic cell sarcoma among clinicians. The clinical and follow-up data of a patient with the right neck FDCS from our hospital were reported, and we have clear the diagnosis and treatment with domestic and foreign literature. The clinical manifestation was a painless mass with slow growth in the right neck and local recurrence after surgery. Microscopic examination showed the structure of the lymph nodes was damaged; tumor cells admixed with abundant lymphocytes in the background arranging in fascicles, whorled, storiform or diffusions. Tumor cells were spindle or oval in shape, with abundant cytoplasm, often visible nucleoli. Immunohistochemical staining showed the patient that S-100（＋）, CD21（＋）, CD23（＋）. Follicular dendritic cell sarcoma is a rare malignant tumor and could be easily misdiagnosed. Due to its low incidence, easily recurrence and metastasis, have something difficulty in imaging diagnosis, using reasonable application of immunohistochemical markers combined with pathological tissue chemistry are the necessary condition to avoid misdiagnosis.
