摘要
目的探讨128层螺旋CT在评价左冠状动脉前降支混合性斑块的狭窄程度与心功能关系中的应用价值。方法将15例单纯性右冠状动脉混合性斑块狭窄患者、100例左冠前降支混合性斑块狭窄患者和50例健康者分别设为研究1组、研究2组和对照组,再根据左冠状动脉前降支的狭窄程度将研究2组分为A、B、C、D四个亚组,所有研究对象均接受128层螺旋CT检查。结果研究1组、研究2组心功能比较无明显差异(P> 0.05),均较对照组差(P <0.05)。研究2组中,A、B组的心功能比较无明显差异(P> 0.05),C、D组心功能与其他三组比较均具有统计学差异(P <0.05),狭窄程度越高的患者心功能越差。结论 128层螺旋CT能够明确左冠状动脉前降支混合性斑块患者的动脉狭窄程度,能够为医师评价患者动脉狭窄程度与心功能关系提供一些客观依据。
Objective To evaluate the value of 128-slice spiral CT in evaluating the relationship between the degree of stenosis of the left anterior descending coronary artery and the cardiac function. Methods Fifteen patients with simple mixed plaque stenosis of the right coronary artery, 100 patients with mixed plaque stenosis of the left anterior descending coronary artery and 50 healthy subjects were selected into study group 1, study group 2 and control group, respectively. According to the degree of stenosis of the left anterior descending coronary artery, the study group 2 were divided into four subgroups： groupA, B, C and D. All subjects were examined by 128 slice spiral CT. Results There was no significant difference in cardiac function between study group 1 and study group 2 （P 〉0.05）, which was worse than that of control group （P 〈0.05）. There was no significant difference in cardiac function between group A and group B （P 〉0.05）, and there was significant difference in cardiac function between group C and group D and other three groups （P 〈0.05）. The higher the degree of stenosis was, the worse the cardiac function was. Conclusion 128-slice spiral CT can identify the degree of arterial stenosis in patients with mixed plaque of left anterior descending coronary artery and provide some objective evidence for physicians to evaluate the relationship between the degree of arterial stenosis and cardiac function.
