中孕期空腹血糖水平与妊娠期糖尿病的相关性研究 被引量:1

摘要 目的探讨中孕期空腹血糖(FPG)水平与妊娠期糖尿病(GDM)相关性研究。方法选取2016年1月—2017年2月在郑州大学第二附属医院产科门诊就诊单胎孕妇977例,按照诊断标准和排除标准共纳入728例。根据OGTT试验结果分为妊娠期糖尿病组(128例)和正常妊娠组(590例)。比较中孕期FPG、孕周和晚孕期75 g口服葡萄糖耐量试验(OGTT)、孕周等。结果 (1)GDM组FPG为(4.85±0.39)mmol/L,正常妊娠组为(4.62±0.32)mmol/L,两组比较,差异有统计学意义(t=-7.289,P<0.05)。(2)FPG最高百分位(181例)GDM(56例)的发生率与最低百分位(182例)的GDM(18例)的发生率比较,OR值为4.082(95%CI 2.286~7.889)。(3)FPG预测GDM的ROC曲线下面积分别为0.665,特异度为55.6%,灵敏度为64.5%。结论中孕期FPG水平的升高,妊娠期糖尿病的发病率增加,FPG预测GDM有一定的预测价值。 Objective To study the correlation between the fasting blood glucose level in the middle pregnancy and gestational diabetes. Methods 977 cases of single-birth delivery women in our hospital from January 2016 to February2017 were selected, 728 cases were included according to the diagnosis standards and exclusion standards, and the patients were divided into the gestational diabetes group with 128 cases and 590 cases in the normal gestational group according to the OGTT test results, and the FPG in the middle pregnancy, pregnancy week and OGTT of 75 g in the late pregnancy were compared. Results 1.The FPG in the GDM group and in the normal gestational group was respectively(4.85±0.39) mmol/L and(4.62±0.32) mmol/L, and the difference was statistically significant(t=-7.289,P0.05). 2.and the comparison of the incidence rate of patients with GDM with the highest percentage FPG(181 cases) and patients with GDM with the lowest percentage FPG(18 cases) showed that the OR value=4.082(95%CI2.286-7.889).3.The lower area of ROC curve of GDM predicted by FPG was 0.665, specificity and sensitivity were respectively 55.6% and 64.5%.Conclusion The FPG level in the middle pregnancy increases, and the morbidity of gestational diabetes increases, and the FPG has a certain predication value in the GDM.
