摘要
目的:观察中药穴位敷贴结合艾灸对维持性血液透析经外周静脉穿刺中心静脉(PICC)置管患者并发症和睡眠质量的影响。方法:按随机数字表法将68例维持性血液透析PICC置管患者分为观察组和对照组,每组34例。两组患者均行艾灸治疗,观察组在对照组基础上行中药穴位敷贴治疗。对治疗前后患者中医症候进行积分,评定患者睡眠质量,观察治疗过程中的并发症发生情况,评价治疗效果。结果:观察组总有效率为70. 59%,对照组总有效率为42. 85%,两组患者总有效率比较差异显著(P<0. 05);两组患者治疗后中医症候积分较治疗前显著降低(P<0. 05),且治疗后观察组中医症状评分显著优于对照组(P<0. 05);观察组患者肺部感染、高血压、心绞痛等并发症的发生率显著低于对照组(P<0. 05);治疗后观察组主观睡眠质量、睡眠时间、日间功能障碍等维度的得分及PSQI总分均显著低于对照组(P<0. 05)。结论:穴位敷贴结合艾灸可有效减少维持性血液透析患者并发症,提高患者睡眠质量,改善中医症状,较单一艾灸治疗效果显著。
Objective： To explore the effect of acupoint application of TCM combined with moxibustion on complications and sleep quality for patients undergoing peripherally inserted central catheter（ PICC） catheterization of maintenance hemodialysis.Methods： 68 patients undergoing PICC catheterization of maintenance hemodialysis were divided into observation group and controlled group by the random number table method,34 cases in each group. All patients were treated with moxibustion,and the observation group was additionally treated with acupoint application of CTM. The TCM symptoms were scored before and after treatment,and the sleep quality of patients was evaluated. The occurrence of complications during the treatment was observed and the therapeutic effect was evaluated. Results： The total effective rates of the two groups were 70. 59% and 42. 85%,respectively（ P〈0. 05）. The TCM symptom scores of the two groups decreased significantly after treatment（ P〈0. 05）,and the scores of the observation group were significantly better than those of the controlled group（ P〈0. 05）. The incidence rate of complications such as pulmonary infection,hypertension and angina pectoris in the observation group was significantly lower than that in the controlled group（ P〈0. 05）. The scores of dimensions such as subjective sleep quality,sleep time and daytime dysfunction and the total score of PSQI of the observation group were significantly lower than those of the controlled group after treatment（ P〈0. 05）. Conclusion： Acupoint application combined with moxibustion can effectively decrease complications of maintenance hemodialysis,improve the sleep quality and TCM symptoms of patients. The curative effect is more significant than that of moxibustion alone.
