不同手术方法治疗Chiari畸形Ⅰ型合并脊髓空洞的效果观察 被引量:4

摘要 目的探讨不同手术方法治疗Chiari畸形Ⅰ型合并脊髓空洞的近远期疗效,为临床治疗提供指导。方法选择我院2010年1月—2015年1月收治的Chiari畸形Ⅰ型合并脊髓空洞患者80例。按手术方法分为观察组和对照组,每组40例。观察组采用改良后颅窝减压+小脑扁桃体电灼或切除+扩大硬膜修补术,对照组采用改良后颅窝减压术+枕大池成形术。观察2组临床疗效及随访远期疗效,脊髓空洞变化情况,记录术后并发症发生情况。结果2组近期疗效和远期疗效、脊髓空洞变化率及术后并发症发生情况比较差异均无统计学意义(P> 0. 05)。结论 2种手术方法治疗Chiari畸形Ⅰ型合并脊髓空洞均能取得较好疗效,临床应根据患者情况选择合适的术式。 Objective To investigate short-ternl and long-term effects of different surgical methods in treatment of patients with type 1 Chiari malformation combined with syringomyelia in order to provide guidance for clinical treat- ment. Methods A total of 80 patients with type I Chiari malformation combined with syringomyelia admitted during Jan- uary 2010 and January 2015 were divided into observation group and control group (u = 40 for each group) according to operative methods. Observation group was treated with modified posterior cranial fossa decompression + fulgurize or excision of amygdala eerebelli + extended dural repair, while control group was treated with modified posterior cranial fossa decompression + eisterna magna plasty. Short-ternl efficacy and long-term follow-up effect were observed, and changes of medullispinal cavity were recorded, and incidence rate of postoperative complications were also recorded in two groups. Results There were no significant differences in shorHerm efficacy, long-term effect, changes of medullispinal cavity or incidence rate of postoperative complications between two groups (P 〉 0.05). Conclusion Both two kinds of operative methods in treatment of patients with type I Chiari malformation combined with syringomyelia may achieve good effects, and therefore clinicians should choose appropriately surgical method according to patient's condition.
