改良“L”型切口对跟骨骨折患者的治疗效果及功能恢复的影响 被引量:5

摘要 目的探讨改良"L"型切口治疗跟骨骨折患者的效果及对患者功能恢复的影响。方法选取我院2015年1月—2016年8月收治的跟骨骨折患者70例,根据手术方法分为改良组35例(改良"L"型切口治疗)、对照组35例(传统"L"型切口治疗)。对比两组的手术时间、出血量、切口长度、手术前后患者踝关节功能、影像学指标变化。结果改良组患者的手术时间、出血量、切口长度均显著的低于对照组,差异有统计学意义(P<0.05);术后6个月,改良组患者的的踝关节功能优65.71%、良25.71%、可8.57%,对照组踝关节功能优45.71%、良34.29%、可20.00%,两组比较差异有统计学意义(P<0.05);术前,改良组和对照组的Bolher角、Gissane角、Peries角和跟骨高度测量值差异无统计学意义(P>0.05);术后6个月,改良组患者的Bolher角显著的高于对照组,Gissane角、Peries角显著的低于对照组,差异有统计学意义(P<0.05)。结论改良"L"型切口治疗跟骨骨折患者的效果优传统L型切口法,有利于患者术后功能恢复。 Objective To investigate the effect of modified"L"incision in the treatment of calcaneal fracture and its effect on functional recovery.Methods 70 patients in our hospital were selected from January 2015 to the time of calcaneal fractures,August 2016,according to the operation method is divided into two groups of 35 patients( modified"L"incision treatment),35 cases in the control group( traditional"L"incision treatment); comparing the two groups of operation time,blood loss,length of incision,surgery patients with ankle joint function and imaging changes.Results The operation time,the modified group of patients with bleeding,incision length were significantly lower than the control group,the difference was statistically significant( P〈0.05); 6 months after the operation,improvement of the ankle function was excellent in 65.71%,good in 25.71% of the patients and control group 8.57%,the ankle function was excellent in 45.71%,good in 34.29% and 20%.There was significant difference between two groups( P〈0.05); preoperative improvement group and control group Bolher angle,Gissane angle,Peries angle and calcaneal height measurements showed no significant difference( P〉0.05); 6 months after surgery,the Bolher angle of the improvement group were significantly higher than the control group( P〈0.05),Gissane angle and the Peries angle was significantly lower than that of control group( P〈0.05).Conclusion The improved L incision is superior to the traditional L incision in the treatment of calcaneal fracture,which is beneficial to the postoperative functional recovery of the patients.
