摘要
目的探讨并分析桥本甲状腺炎合并甲状腺乳头状癌患者的超声表现及颈部淋巴结转移的关系。方法分析109例HT合并PTC患者共131个PTC结节的术前超声表现,根据病理有无颈部淋巴结转移分为转移组42例和对照组67例,对PTC超声表现与颈部淋巴结转移进行单因素研究,通过Logistic回归,分别以病理诊断颈部淋巴结转移和甲状腺乳头状癌超声特征为方程中的因变量和自变量,对该方程的预报能力进行评估。结果通过单因素和Logistic回归分析后发现,甲状腺被膜侵犯、微钙化、癌结节最大直径、边界与颈部淋巴转移有关系(P <0. 05)。癌结节纵横比、血流分级Ⅱ~Ⅲ级对预测颈部淋巴结转移无统计学意义。结论癌结节最大直径、边界、甲状腺被膜侵犯、微钙化能够成为术前评估PTC能否存在颈部淋巴结转移的重要依据。
Objective To explore the ultrasonic manifestations of Hashimoto＇s thyroiditis complicated with thyroid papillary carcinoma,and to analyze its relationship with cervical lymph node metastasis. Methods Preoperative ultrasonic findings of 109 patients with 131 PTC nodules were analyzed retrospectively. According to pathology of cervical lymph node metastasis,patients were divided into metastasis group（n = 42） and control group（n = 67）. The single factor of thyroid ultrasound and cervical lymph node metastasis,pathologic diagnosis performance cervical lymph node metastasis and thyroid ultrasound were used as variables. Logistic regression model was established. The prediction ability of the model was evaluated. Results Single factor analysis and multiple factor analysis found that the maximum diameter,boundary,and the incidence of the invasion of the thyroid gland and the micro calcification of the tumor had correlation with cervical lymph node metastasis,which was statistically significant（P 〈0. 05）. There was no statistical significance in the prediction of cervical lymph node metastasis in cancer nodules aspect ratio and blood flow grade Ⅱ ～ Ⅲ level（P〉 0. 05）. Conclusion The tumor nodule maximum diameter,border,thyroid capsule invasion and micro calcification can evaluate of PTC with cervical lymph node metastasis.
