摘要
目的评价泻心汤治疗上消化道出血的疗效。方法检索中国生物医学数据库、中国知网、万方数据库、维普、Pub Med,搜集泻心汤对照临床常用药物治疗上消化道出血的临床随机对照试验。提取资料并运用Cochrane风险偏倚评估工具进行风险评估。采用Rev Man5.3软件进行异质性检验、敏感性分析、合并效应量等分析。结果共纳入29个试验,共计4098例患者。临床总有效率Meta分析结果显示:泻心汤加减对照西药组在临床总有效率方面差异有统计学意义[RR=1.12,95%CI(1.07,1.17)],P<0.00001。亚组分析结果:1)"泻心汤加减+人工合成生长抑素vs质子泵抑制剂+人工合成生长抑素组"在临床总有效率方面差异有统计学意义[RR=1.20,95%CI(1.04,1.38)],P=0.01;2)"泻心汤加减+质子泵抑制剂vs H2受体阻滞剂+垂体后叶素组"在临床总有效率方面差异有统计学意义[RR=1.39,95%CI(1.20,1.62)],P<0.0001;3)"泻心汤加减+质子泵抑制剂vs质子泵抑制剂组"在临床总有效率方面差异有统计学意义[RR=1.16,95%CI(1.01,1.32)],P=0.03。结论泻心汤对照西药在临床总有效率方面有较显著的优势,泻心汤联合西药对照西药组也体现了疗效优势。但上述结论尚需更多高质量的随机双盲对照试验加以验证。
Objective： To assess the therapeutic effect of Xiexin Decoction on upper gastrointestinal bleeding. Methods： Xiexin Decoction versus common clinical drugs in the treatment of upper gastrointestinal bleeding were identified fi＇om CBM,CNKI,Wanfang Database,VIP and Pubmed. Data extraction and Coehrane risk bias assess- ment tool were used for risk assessment. RevMan5.3 software was used for heterogeneity test,sensitivity analysis and combined effect analysis. Results： 29 trials involving 4098 patients were included. The results of recta-anal- yses showed that there was a significant difference in the total clinical efficiency among groups. Conclusion： Xiexin Decoction is superior to common clinical drugs alone in enhancing the total effective rate. Further large scale random comparison trials are required to validate the role of Xiexin Decoction on upper gastrointestinal bleeding.
