摘要
目的探讨动脉血乳酸检测在新生儿窒息后器官损害程度及预后判断中的临床价值。方法选择74例新生儿窒息患儿作为研究对象,同期选择40例正常新生儿作为对照组。新生儿窒息患儿根据1 min Apgar评分分为重度窒息组(32例)和轻度窒息组(42例);根据多器官损害诊断标准分为单器官损害组(40例)和多器官损害组(34例);根据预后分为治愈组(63例)和死亡组(11例)。比较各组血乳酸水平、动脉血气指标,并分析相关性关系。结果各组血乳酸水平与1 min Apgar评分、血p H值、BE呈负相关性关系(r=-0. 675,-0. 408,-0. 512,P <0. 05);多器官损害组血乳酸水平明显高于单器官损伤组(P <0. 05),而1 min Apgar评分、血p H值、BE均明显低于单器官损伤组(P <0. 05);治愈组与死亡组血p H值、BE比较无显著性差异(P> 0. 05),而死亡组1 min Apgar评分明显低于治愈组(P <0. 05),血乳酸水平明显高于治愈组(P <0. 05)。结论动脉血乳酸在新生儿窒息患儿器官损害程度及预后判断中具有较高的临床价值。
Objective To investigate the clinical value of arterial blood lactate detection in determine of organ dam- age and prognosis after asphyxia neonatorum. Methods 74 neonates with asphyxia neonatorum were selected as the sub- jects, and 40 normal newborns were selected as the control group at the same time. Children with neonatal asphyxia were di- vided into severe asphyxia group (32 cases) and mild asphyxia group (42 cases) according to the 1 rain Apgar score, were divided into single organ damage group (40 cases) and multiple organ damage group (34 cases) according to the diagnostic criteria of multiple organ damage, and were divided into cure group (63 cases) and death group ( 11 cases) according to the prognosis. The Blood lactic acid level and arterial blood gas indexes of evetry- group were compared, and the correlation was analyzed. Results The blood lactic acid level of every- groups was negatively con'elated with 1 rain Apgar score, blood pH, BE (r = -0.675, -0.408, -0.512, P 〈0.05). The blood lactic acid level in the multiple organ damage group was sig- nificantly higher than that of the single organ damage group ( P 〈 0.05 ) , but 1 rain Apgar score, blood pH value, BE were significantly lower than that of single organ injury- group ( P 〈 0.05 ). There was no significant difference between the cured group and the death group about the blood pH and BE (P 〉0.05) , but the hnin Apgar score of the death group was signifi- cantly lower than that of cure group( P 〈 0.05 ), the level of blood lactic acid was obvious higher than that of the cure group (P 〈 0.05 ). Conclusion Arterial lactate has a high clinical value in predicting organ damage and prognosis in neonates with asphyxia.
