摘要
目的探讨宫颈上皮内瘤变(Cervical Intraepithelial Neoplasia,CIN)病变切缘阳性的影响因素。方法选取临沂市妇女儿童医院2016年1月至2017年12月收治的115例行手术治疗的CIN患者作为研究对象,术后依据切缘性质分为切缘阴性组100例及切缘阳性组15例。统计两组患者的基本资料,包括年龄、临床症状、病程、CIN分级、碘不着色区、是否绝经、宫颈外观、手术方式,并对影响子宫颈CIN病变切缘阳性的因素进行总结。结果两组患者年龄、临床症状、病程比较,差异均无统计学意义(均P>0.05),而CIN分级、碘不着色区、是否绝经、宫颈外观、手术方式比较,差异均有统计学意义(均P<0.05);Logistic回归分析结果提示,CIN分级Ⅲ级、绝经后、碘不着色区>3 cm、宫颈环形电刀切除术为子宫颈CIN病变切缘阳性的独立风险因素(OR:7.885、5.347、5.340、4.337,95%CI:3.374~7.510、2.859~5.453、2.777~5.450、2.324~4.897)。结论 CIN分级Ⅲ级、绝经后、碘不着色区>3 cm、宫颈环形电刀切除术为子宫颈CIN病变切缘阳性的独立风险因素,临床应予以足够的重视并及时采取补救措施。
Objective To analyze factors influencing the positive margin of cervical intraepithelial neoplasia Methods 115 cases of CIN patients treated in Linyi women＇s and children＇s Hospital from January 2016 to December 2017 were selected as the research subjects. After the operation, 100 cases were divided into negative group and 15 cases were positive by cutting edge. The basic data of the two groups were collected, including age clinical symptoms, course of disease, CIN classification, iodine non coloring area, menopause, cervical appearance and surgical methods, and the factors affecting the positive margin of cervix CIN lesions were summarized. Results Two groups of age, symptoms, duration of time, there was no statistically significant difference （P〉0.05）, CIN grade odine not shaded area, menopausal status, cervical appearance, operation method, the difference was statistically significant （P〈0.05）; Logistic regression analysis results suggest that CIN class Ⅲ, postmenopausal, iodine is not shaded area 〉 3 cm for cervix, cervical loop electric knife resection of CIN lesions cut edge positive factors （OR： 7.885, 5.347, 5.340, 4.337, 95% CI： 3.374-7.510, 2.859-5.453, 2.777-5.450, Conclusion CIN class Ⅲ, postmenopausal, iodine is not shaded area 〉 3 cm for cervix, cervica resection of CIN lesions cut edge positive independent risk factors, clinical enough concern must remedial measures in time.
