摘要
目的探究分析早期肠内营养对ICU呼吸机治疗患者的营养水平改善及临床预后的影响。方法方便选取自2015年1月—2018年3月该院收治的予以呼吸机治疗的ICU危重患者72例作为此次研究对象,随机分为对照组和观察组,每组36例。其中,观察组患者在入院的48 h内给予肠内营养,给予对照组患者肠外营养。治疗周期结束后比较两组患者治疗前后血红蛋白、血浆白蛋白及总蛋白等营养指标的差异以及两组患者治疗后的并发症发生情况。结果治疗周期结束后,两组患者的营养指标均得到了不同程度的改善,但观察组患者血红蛋白治疗后为(120.5±11.3)g/L vs对照组患者的(103.3±10.5)g/L(t=8.32);血浆白蛋白为(39.5±4.5)g/L vs对照组患者的(32.7±4.3)g/L(t=5.73);总蛋白为(67.8±5.1)g/L vs对照组患者的(61.5±3.8)g/L(t=5.38)。组间比较差异有统计学意义(P<0.05)。观察组患者营养治疗结束后并发症的发生率为22.22%,显著低于对照组患者的61.11%,组间比较差异有统计学意义(χ~2=18.03,P<0.05)。结论对ICU呼吸机治疗患者给予早期肠内营养可显著改善患者的营养状态,显著提高患者的机体免疫功能,值得在临床上加以推广和应用。
Objective To investigate the effects of early enteral nutrition on nutritional improvement and clinical prognosis in patients treated with ICU ventilator. Methods 72 ICU critically ill patients who underwent ventilator treatment from January 2015 to March 2018 were randomly divided into the control group and the observation group, with 36 cases in each group. Among them, patients in the observation group were given enteral nutrition within 48 hours after admission, and parenteral nutrition was given to the control group. At the end of the treatment period, the differences in nutritional indicators such as hemoglobin, plasma albumin and total protein between the two groups were compared before and after treatment, and the complications of the two groups were analyzed. Results After the end of the treatment period, the nutritional indexes of the two groups were improved to different extents, but the patients in the observation group were (120.5±11.3)g/L after treatment with hemoglobin (103.3±10.5)g/L(t=8.32); plasma albumin was (39.5±4.5)g/L and that of the control group (32.7±4.3)g/L(t=5.73); total protein was (67.8±5.1)g/L and the control group of patients (61.5±3.8)g/L(t=5.38). The difference between the groups was statistically significant (P〈0.05). The incidence of complications after the end of nutritional therapy in the observation group was 22.22%, which was significantly lower than that in the control group (61.11%). The difference between the two groups was statistically significant (X^2=18.03, P〈0.05). Conclusion Early enteral nutrition in patients treated with ICU ventilator can significantly improve the nutritional status of patients and significantly improve the immune function of patients. It is worthy of clinical promotion and application.
