法医评定肺压缩比率MDCT方法研究 被引量:2

摘要 目的探讨气胸、液气胸中的肺压缩比率及气胸分度的法医伤情评定MDCT新方法。方法应用MDCT重建技术测量被压缩肺组织及胸腔内气体、液体的体积,并与以往X线分度法进行比较,确定胸腔内气体、液体含量及气胸分度的具体法医评定新方法。结果分别测得胸腔内气体、液体体积以及被压缩的肺组织体积,求得胸腔内气体与该气体和被压缩肺组织体积之和的比值,即G/G+L(气胸患者),或求得胸腔内气体与该气体和胸腔内液体体积、被压缩肺组织体积之和的比值,即G+E/G+E+L(液气胸患者),计算得出肺压缩比率,同时依据X线分度标准确定气胸分度:I度气胸≤25%,II度气胸> 25%并<50%,III度气胸≥50%,进而运用卡方检验检测X线法和CT法所算得的肺压缩百分比在气胸分度上是否有差异性,另对X线法和CT法所算得的肺压缩百分比进行相关性分析。结论 MDCT重建技术测量肺压缩比率更快捷,对气胸分度更加精确,为气胸法医学鉴定的伤情评定提供了可靠的诊断依据。 Objective To explore a new method for determining the compression ratio of lung in pneumothorax and liquid pneumothorax, and determining the grade of pneumothorax in forensic identification by MDCT. Methods MDCT reconstruction technique was applied to measure the volume ofthe gas and the liquid in the compressed lung in anocelia, then, the result were compared with those in pneumothorax by x-ray measurement, the new method of forensic identification for the grade of pneumothorax and measurement of the volume of the gas and the liquid in anocelia was determined. Results The chest cavity gas, liquid volume and the compressed lung volume were respectively measured, the ratio of the gas to the sum of the volume of the gas and compressed lung was obtained as G/ G+L (patients with pneumothorax), or the ratio of the gas to the sum of the volume of the gas, the liquid and compressed lung was obtained as G+E/G+E+L (patients with liquid pneumothorax). Then, the pneumothorax grade was determined as follows according to the X-ray standard: less than or equal to 25% was grade Ⅰ , more than 25% and less than 50% was grade Ⅱ , more than or equal to 50% was grade Ⅲ. Then chi-square test was used to determine whether there is difference in compression percentages calculated by X-ray method and CT method in pneumothorax, the next correlation analysis was done. Conclusion MDCT reconstruction technique is a more efficient way for measuring the compression ratio of lung, and more accurate for grading the pneumothorax. It provides a reliable diagnostic basis for the forensic identification of pneumothorax.
