摘要
目的系统评价双歧杆菌四联活菌片辅助治疗幽门螺杆菌(Hp)相关消化性溃疡或慢性胃炎的疗效。方法通过计算机检索万方、CNKI、CBM、Pub Med数据库,检索时限从建库至2017年10月。搜集国内外公开发表的关于双歧杆菌四联活菌片治疗Hp感染相关的随机对照试验(RCT),采用改良Jadad量表进行质量评价,采用Rev Man 5.3软件进行Meta分析。结果纳入文献22篇,包含Hp阳性的消化性溃疡或慢性胃炎患者2 799例。Mata分析结果显示:(1)试验组(双歧杆菌四联活菌片联合PPI三联疗法) Hp根除率、症状缓解率和消化性溃疡愈合总有效率均高于三联疗法对照组,不良反应发生率和复发率均低于三联疗法对照组,差异均有统计学意义(P <0.05)。(2)试验组(双歧杆菌四联活菌片联合含铋四联疗法) Hp根除率高于四联疗法对照组,不良反应发生率和复发率低于四联疗法对照组,差异均有统计学意义(P <0.05); 2组消化性溃疡愈合总有效率比较差异无统计学意义(P>0.05)。结论双歧杆菌四联活菌片联合三联或四联疗法均能提高Hp根除率,降低不良反应发生率和复发率,值得临床应用。
Objective To systematically evaluate the efficacy of Bifidobacterium tetravaccine tablets in the treatment of Helicobacter pylori associated peptic ulcer or chronic gastritis.Methods The Wanfang,CNKI,CBM,and Pub Med databases were retrieved by computer,and the search time was from the database construction to October 2017.A randomized controlled trial（RCT） related to the treatment of Hp infection by Bifidobacterium quadruple live bacteria tablets was collected at home and abroad.After quality evaluation using the modified Jadad scale,Meta analysis was performed using Rev Man 5.3 software.Results 22 articles were finally included,including 2 799 patients.The results of mata-analysis showed：（1） The Hp eradication rate,symptom relief rate and peptic ulcer healing rate in the experimental group（Bifidobacterium quadruple live bacteria combined with PPI triple therapy） were higher than those in the control group（triple therapy）,the incidence of adverse reactions and recurrence rate in the experimental group were lower than those in the control group,the differences were statistically significant（P〈0.05）.（2） The Hp eradication rate of the experimental group（Bifidobacterium quadruple live bacteria combined with sputum-containing quadruple therapy） was higher than that of the quadruple therapy group,the incidence of adverse reactions and recurrence rate in the experimental group were lower than those in the quadruple therapy group,the differences were statistically significant（P〈0.05）.There was no significant difference in the healing rate of peptic ulcer between the two groups（P〉0.05）.Conclusion Bifidobacterium quadruple live bacteria combined with triple or quadruple therapy can improve Hp eradication rate,reduce the incidence of adverse reactions and recurrence rate,and it is worthy of clinical application.
