穿心莲内酯注射液对急性支气管肺炎患儿体温的降低作用及机制研究 被引量:2

摘要 目的探讨穿心莲内酯注射液对急性支气管肺炎患儿体温降低作用及机制研究。方法选取106例急性支气管肺炎儿童患者,按照随机数表法将其分为对照组与治疗组,每组53例。所有患儿均进行常规治疗,当患儿体温超过38. 5℃时,给予补热息痛或布洛芬对症治疗,治疗组在此基础上给予穿心莲内酯注射液静脉滴注治疗。比较两组患儿的体温平稳时间,监测并比较治疗前后两组患儿血清白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)、前列腺素E2(PGE2)、γ干扰素(IFN-γ)及白细胞介素6(IL-6)变化。结果治疗组体温平稳时间为(3. 21±0. 26) d,低于对照组的(4. 27±0. 24) d,差异有统计学意义(t=21. 809,P <0. 001)。治疗前两组患者IL-1β、TNF-α、PGE2、IFN-γ及IL-6指标比较差异未见统计学意义(P> 0. 05);治疗后,治疗组患者PGE2、IFN-γ及IL-6指标较治疗前显著改善(P <0. 05)。治疗组PGE2与IFN-γ指标较对照组改善(P <0. 05)。结论急性支气管肺炎患儿应用穿心莲内酯注射液可以抑制PGE2释放,迅速降低患儿体温,并维持体温平稳。其机制为穿心莲内酯注射液抑制IL-6和IFN-γ等内源性致热源,抑制下丘脑释放PGE2,达到降低并维持体温平稳的作用。 Objective To evaluate the effects of andrographolide injection on body temperature reduction in children with acute bronchial pneumonia and its related mechanism. Methods A total of 106 children with acute bronchial pneumonia were selected. According to the random number table, the children were divided into the treatment group and the control group, with 53 cases in each group. All compared. Serum levels of interleukin-1β ( IL-1β) , tumor necrosis factor α ( TNF-α), prostaglandin E2 (PGE2), gamma interferon(IFN-γ) and interleukin 6 (IL-6) were measured and compared before and after treatment. Results The time of body temperature stabilization in the treatment group was significantly lower than that in the control group [ (3.21 ±0. 26) d vs. (4.27 ±0. 24) d, t =21. 809, P 〈 0. 001 ]. There was no significant difference in the levels of IL-1β, TNF-α, PGE2, IFN-γ and IL-6 before treatment between the two groups ( P 〉 0. 05 ). After treatment, the PGE2, IFN-γ and IL-6 in the treatment group were improved compared with those before treatment (P 〈 0. 05 ). Compared with the control group, the indexes of PGE2 and IFN-γ in the treatment group were improved ( P 〈 0. 05 ). Conclusions The use of andrographolide injection in children with acute bronchial pneumonia can inhibit the release of PGE2, rapidly reduce the body temperature of children, and maintain a stable body temperature. The mechanism is the inhibition of endogenous pyrogens such as IL-6 and IFN-γ by andrographolide injection, inhibiting the release of PGE2 from the hypothalamus, and reducing and maintaining the effect of stable body temperature.
