清眩润目饮对蒸发过强型干眼模型鼠角结膜炎症因子表达的影响 被引量:11

Effects of Qingxuan Runmu decoction on expression of inflammation factors in cornea and conjunctiva of rat model with high evaporation dry eye

摘要 目的探讨清眩润目饮对蒸发过强型干眼模型鼠角、结膜炎症因子表达的影响。方法将18只SD大鼠随机分为3组,空白对照组、模型组、清眩润目饮组,空白对照组不做任何处理,其余2组采用纤维电针改良的灼烧器直接灼烧模型鼠睑板腺开口造模;清眩润目饮组予中药清眩润目饮灌胃qd,模型组以等量生理盐水灌胃qd,连续灌胃28 d。各组分别于造模前、造模后即刻、造模后4 w行角膜荧光素染色(CFS)、泪膜破裂时间(BUT)、泪液分泌量检查(SIT)。造模后28 d处死动物,并进行角、结膜炎性因子水平及相关蛋白含量测定及用光镜观察角、结膜组织病理学改变。结果 (1)CFS、BUT、SIT:造模后4 w,清眩润目饮组的CFS评分、BUT、SIT均优于模型组(P<0.05),且结果与同期空白对照组接近(P>0.05)。(2)各组大鼠结膜组织炎性因子IL-1β、IL-6、TNF-α表达量比较:模型组大鼠IL-1β、IL-6、TNF-α较空白对照组大鼠数值明显升高(P<0.01);清眩润目饮组较模型组IL-1β、IL-6、TNF-α降低(P<0.01);清眩润目饮组较空白对照组,无统计学显著性(P>0.05)。(3)各组大鼠结膜组织TLR4和MYD88蛋白表达量比较:模型组大鼠TLR4、MyD88蛋白较空白对照组大鼠数值明显升高(P<0.01);清眩润目饮组较模型组降低(P<0.01)。(4)病理组织学检查:空白对照组实验鼠角、结膜组织内未见明显炎性细胞浸润;模型组、清眩润目饮组实验鼠角、结膜组织中均有不同程度的炎性细胞的表达,中药清眩润目饮组角、结膜炎性细胞表达明显弱于模型组。结论清眩润目饮可以减轻睑板腺功能障碍所致蒸发过强型干眼角、结膜的炎性反应,降低炎性因子IL-1β、IL-6、TNF-α的含量,影响TLR4、MyD88蛋白质在角、结膜中的表达,其对干眼的治疗作用可能与此有关。 OBJECTIVE To investigate the effect of Qingxuan Runmu decoction on expres- sion of inflammatory cytokines in rat cornea and conjunctiva. METHODS The 18 SD rats were randomly divided into 3 groups: blank control group, model group, Qingxuan Runmu decoction group. Blank control group did not accept any intervention,the other two groups were applied with modified fibre-electric needle, and the model was directly cauterized;Qingxuan Runmu decoction group was given a daily dose of Chinese medicine,while the model group was treated with equivalent amount of saline. Continuous gavage lasted for 28 d. In each group, the corneal fluorescein staining (CFS), tear film rupture time (BUT) and tear secretion test (SIT) were performed before modeling, immediately after modeling and 4 weeks afterward. The animals were killed at the 28th day. The level of inflammatory cytokines and relative protein content were measured, and the pathological changes of the corneal and conjunctival tissue were observed. RESULTS (1)Results of CFS, BUT, SIT: Four weeks after the modeling, the CFS score, BUT and SIT in the Qingxuan Runmu decoction group were better than the model group (P〈0.05), and the results were close to counterparts of the blank control group (P〉0.05). (2)Comparison of expression levels of inflammatory cytokines IL-1 β, IL-6, TNF-α between groups:The rats in the Qingxuan Runmu decoction group showed significantly higher IL-1 β, IL-6, TNF-α level than in the blank control group (P〈0.01). While IL-1 β, IL-6, TNF-α in the Qingxuan Runmu decoction group were lower than model group(P〈0.01 ). Comparison between the blank control group and the Qingxuan Runmu decoction group showed no statistically signifi- cance(P〉0.05). (3)Comparison of expression of TLR4 and MYD88 protein in rat conjunctival tis- sue:The TLR4 and MyD88 in model group was significantly higher than that in the blank control group (P〈0.01). The result of Qingxuan Runmu decoction group was lower than the model group (P〈0.01). (4)Histopathological examination: No obvious inflammatory cell infiltration was observed in the rat corneal and conjunctival tissue in the blank control group. The expressions of inflammatory cells in the rat corneal and conjunctival tissue were expressed in the model group and the Qingxuan Runmu decoction group. The expressions of inflammatory cells in the Qingxuan Runmu decoction group were significantly lower than that in the model group. CONCLUSIONS Qingxuan Runmu decoction alleviated corneal and conjunctival inflammatory reaction in evaporative dry eye owing to meibomian gland dysfunction. It reduced levels of inflammatory factors as I L-6, IL-1β the content of TNF-α, influenced TLR4, MyD88 protein expression in the cornea and con- junctiva, which might be the underlying mechanism of its effect on dry eye.
