股骨近端防旋髓内钉治疗老年股骨粗隆间骨折的临床预后分析 被引量:5

摘要 目的探讨股骨近端防旋髓内钉治疗老年股骨粗隆间骨折的临床效果及预后。方法选取2013年6月~2015年10月期间获得随访的90例老年股骨粗隆间骨折患者作为研究对象,根据手术方法分组。对照组45例患者进行股骨近端伽玛型髓内钉(PFNA)内固定治疗,观察组45例患者行股骨近端联合加压交锁髓内钉(Inter Tan)治疗。比较两组老年股骨粗隆间骨折手术效果;手术开展时间、切口平均长度、手术中总失血情况、股骨粗隆间骨折愈合时间;手术前后患者功能指标、疼痛指标;并发症等情况。结果随访时间6~18个月,平均13个月,观察组老年股骨粗隆间骨折手术效果高于对照组,差异有统计学意义(P <0.05);观察组患者手术开展时间、手术中总失血情况多于对照组;切口平均长度、疼痛指标与对照组相似;股骨粗隆间骨折愈合时间略短于对照组;手术前两组功能指标相近;手术后观察组患者功能指标优于对照组,差异有统计学意义(P<0.05)。观察组并发症少于对照组,差异有统计学意义(P <0.05)。结论老年股骨粗隆间骨折患者行股骨近端联合加压交锁髓内钉(Inter Tan)较股骨近端伽玛型髓内钉(PFNA)治疗,骨折复位效果及维持骨折解剖对位确实可靠,可减少术中及术后骨折再移位、复位丢失,缩短患者康复时间,加速骨折愈合,改善关节功能。
