摘要
目的分析80例接受全结肠系膜切除术的结肠癌患者术后5年无进展生存情况及其影响因素。方法回顾性分析2011年5月至2012年5月本院80例行完整结肠系膜切除术的结肠癌患者临床病历资料,术后进行随访,记录5年无进展生存情况及其影响因素。结果术后5年失访3例,无进展生存51例,无进展生存率为63.75%。单因素分析结果显示,5年无进展生存组与肿瘤进展组间肿瘤分期、淋巴结转移情况、分化程度及肿瘤切缘性质差异均有统计学意义(均P<0.05)。Cox风险回归模型分析结果显示,肿瘤分期、淋巴结转移及分化程度是患者术后5年无进展生存的独立影响因素(均P<0.05)。结论完整肠系膜切除术后患者5年无进展生存率为63.75%,肿瘤分期情况、淋巴结转移情况和分化程度是完整结肠系膜切除术后患者5年进展生存情况的独立影响因素。
Objective To analyze five-year progression-free survival in 80 patients with colon cancer after complete mesocolic excision and to investigate risk factors for survival. Methods Clinical data of 80 patients with colon cancer who received complete mesocolic excision in our hospital between May 2011 and May 2012 were retrospectively analyzed. Patients were followed-up for five years. Five-year survival status was recorded and risk factors for survival were analyzed. Results At 5 years, 3 patients were lost to follow-up, 51 were progression-free. Five-year progression-free survival rate was 63.75%. Univariate analysis showed that stage of tumor, lymph node metastasis, degree of differentiation, and characteristics of excision margin of tumor were significant risk factors for five-year progression-free survival （P 〈 0.05）. Cox regression analysis showed that stage of tumor, lymph node metastasis, and degree of differentiation independent risk factors for five-year progression-free survival （P 〈 0.05）. Conclusion Five-year progression-free survival for patients after complete mesocolic excision was 63.75%. Stage of tumor, lymph node metastasis, and degree of differentiation were risk factors for five-year progression-free survival after complete mesocolic excision.
