数字化靶点单侧穿刺与传统双侧穿刺经皮椎体后凸成形术治疗老年骨质疏松性椎体压缩性骨折的对比研究 被引量:3

摘要 目的:对比研究数字化靶点单侧与传统双侧穿刺经皮椎体后凸成形术治疗老年骨质疏松性椎体压缩性骨折的临床疗效。方法:回顾分析2015年1月-2017年1月,新疆生产建设兵团第四师医院确诊为老年骨质疏松性椎体压缩性骨折患者100例,分为数字化靶点单侧穿刺组和传统双侧穿刺组,每组各50例,均行经皮椎体后凸成形术,随访并比较两组患者手术时间、骨水泥注入量及X线透视次数,比较术前、术后2天、术后第6个月及术后第12个月疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)变化。结果:所有患者均获得随访,随访时间6个月~24个月,平均15个月,术后症状均得到不同程度的缓解;单侧组的手术时间、X线透视次数、骨水泥注入量明显低于双侧组,差异有统计学意义(P<0.05);两组患者VAS、ODI评分较术前均明显降低,差异有统计学意义(P<0.05),同一时间两组间比较差异无统计学意义(P>0.05)。结论:数字化靶点单侧穿刺与传统双侧穿刺经皮椎体后凸成形术治疗老年骨质疏松性椎体压缩性骨折临床疗效均满意,数字化靶点单侧穿刺手术时间短,X线透视次数少,安全性高,操作过程简化,值得推广。 Objective: To compare the clinical effects of digital target unilateral and traditional bilateral percutaneous kyphoplasty in the treatment of osteoporotic vertebral compression fractures in the elderly. Methods:100 patients with osteoporotic vertebral compression fractures in the four division hospital of Xinjiang corps From January 2015 to January 2017 were divided into digital target single side puncture group and traditional bilateral puncture group,with 50 cases in each group.All the patients were treated with percutaneous kyphoplasty,followed up and compared the operation time and bone cement in the two groups.The changes of pain visual analogue score (VAS) and Oswestry dysfunction index (ODI) were compared before,2 days after operation,sixth months after operation and twelfth months after operation. Results: All patients were followed up for 6 months to 24 months,with an average of 15 months.All the symptoms were relieved after operation.The operative time,X-ray fluoroscopy times and bone cement injection volume in unilateral group were significantly lower than those in bilateral group(P〈0.05).The scores of VAS and ODI in the two groups were significantly lower than those before the operation,and the difference was statistically significant(P〈0.05).There was no significant difference between the two groups at the same time(P〉0.05). Conclusion: The clinical effect of digital target unilateral puncture and traditional bilateral puncture percutaneous kyphoplasty are satisfactory in the treatment of osteoporotic vertebral compression fractures in the elderly.Digital target unilateral puncture has shorter operation time,fewer X-ray fluoroscopy times,high safety and simplified operation process,which is worth promoting.
