高分辨率扩散加权成像在青少年原发性骨肉瘤中的价值研究

The value of high resolution diffusion-weighted imaging in adolescent primary osteosarcoma

摘要 目的探讨高分辨率扩散加权成像(RESOLVE-DWI)对青少年原发性骨肉瘤诊断价值,并比较其与常规DWI(EPI-DWI)的图像质量和表观扩散系数(ADC)值的差异。方法采用Sie-mens Skyra 3. 0T超导MRI扫描仪,分别采集30例疑似青少年原发性骨肉瘤患者病变部位的MR图像,均行RESOLVE-DWI+EPI-DWI序列扫描。并对图像质量进行评分比较,同时测量骨肉瘤组织图像的信噪比(SNR)及ADC值进行比较,并与最后病检结果相对照。用两独立样本t检验法比较不同扩散加权成像序列的各项图像质量值评分、SNR、ADC值。结果 30例受检者,RESOLVE-DWI序列图像整体质量明显优于EPI-DWI弥散图像,其中RESOLVE-DWI评分≥4分者23例,EPI-DWI评分≥4分者1例,差异有统计学意义(P <0. 01); RESOLVE-DWI序列组和EPI-DWI序列组的骨肉瘤组织平均ADC值分别为(856. 16±21. 06)×10-3mm2和(824. 49±33. 45)×10-3mm2,差异无统计学意义(P> 0. 05); RESOLVE-DWI序列组的SNR值明显高于EPI-DWI序列组,分别为3. 77±0. 51和1. 97±0. 23,差异有统计学意义(P <0. 01)。结论 RESOLVE-DWI技术应用于扩散加权成像序列能改善图像质量,提高信噪比,并且对骨肉瘤组织ADC值的测量没有影响,能为青少年原发性骨肉瘤诊断提供更有价值的诊断效能,可以常规用于青少年原发性骨肉瘤扫描。 Objective To evaluate the diagnostic value of high resolution diffusion weighted ima- ging (RESOLVE-DWI) in adolescent primary osteosarcoma, and to compare the difference of image quality and apparent diffusion coefficient (ADC) value with conventional DWI echo planar imaging (EPI)-DWI. Methods Siemens Skyra 3.0T superconducting magnetic resonance imaging ( MRI ) scanner was used to collect MRI images of 30 patients suspected to be adolescent primary osteosarcoma. RESOLVE-DWI EPI- DWI was performed. Meanwhile, the signal-to-noise (SNR) and ADC values of osteosarcoma tissues were measured and compared with the final pathological examination results. The image quality scores, SNR and ADC values of different diffusion weighted imaging sequences were tested by two independent samples t test. Results The overall quality of RESOLVE-DWI sequence images was significantly better than that of EPI- DWI diffusion images, including 23 patients with RESOLVE-DWI scores 〉/ 4 scored and 1 patients with EPI-DWI seores≥4 ,with significant difference( P 〈 0. 01 ). The mean ADC values of osteosarcoma tissues in RESOLVE-DWI sequence group and EPIDW sequence group were (856. 16 + 21.06) × 10 ^-3 mm^2 and (824.49 +-33.45) ×10^-3 mm^2 ,with no significant difference (P 〉0. 05). The average SNR of RESOLVE- DWI group was significantly higher than that of EPI-DWI sequence group ( 3.77 +- 0. 51 vs 1.97 + 0. 23, P 〈 0. 01 ). Conclusions RESOLVE-DWI can improve the image quality and the SNR, and the ADC val- ue of osteosarcoma tissue is not affected. It can provide more valuable diagnostic efficacy for the diagnosis of primary osteosareoma in adolescents, and can be routinely used in the scanning of primary osteosarcoma in adolescents.
