牙周组织再生术联合口腔正畸治疗中重度牙周炎的临床疗效 被引量:37

摘要 目的探讨牙周组织再生术联合口腔正畸治疗中重度牙周炎的临床疗效。方法选取中重度牙周炎患者54例,根据治疗方式分为观察组和对照组各27例。对照组患者仅给予牙周组织再生术,观察组在对照组基础上联合口腔正畸治疗。比较2组患者临床疗效、治疗前后血清和龈沟液中炎症因子水平变化及牙周情况。结果观察组总有效率为92. 59%,显著高于对照组的70. 37%(P <0. 05)。与治疗前相比,治疗3个月后2组血清和龈沟液中白介素-5(IL-5)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)水平显著降低,且观察组显著低于对照组(P <0. 01)。治疗3个月后2组患者牙菌斑指数(PLI)、牙龈指数(GI)、龈沟出血指数(SBI)、临床附着丧失(AL)及牙周探诊深度(PD)均较治疗前显著降低,且观察组各指标显著低于对照组(P <0. 01)。结论中重度牙周炎患者行牙周组织再生术联合口腔正畸治疗可明显减轻炎症反应,改善其牙周情况。 Objective To investigate the clinical efficiency of periodontal tissue regeneration combined with orthodontics in treatment of patients with moderate and severe periodontitis. Methods Totally 54 patients with moderate and severe periodontitis were selected and divided into observation group and control group according to therapeutic methods, with 27 cases in each group. The patients in the control group were treated with periodontal tissue regeneration, while the observation group was treated with orthodontic treatment on the basis of the control group. The clinical efficiency, the levels of inflammatory factors in serum and gingival crevicular fluid, and periodontal conditions were compared between the two groups before and after treatment. Results The clinical effective rate of the observation group was 92.59%, which wassignificantly higher than 70.37% of the control group ( P 〈 0.05). After 3 months of treatment, the levels of IL-5, TNF-α and IL-1β in the serum and gingival crevicular fluid were significantly lower than those before the treatment in both groups, and the levels of these indexes above in the observation group were significantly lower than those in the control group ( P 〈0.01). After 3 months of treatment, the PLI, GI, SBI, AL and PD were significantly lower than those before treatment in both groups, and the values of indexes above in the observation group were significantly lower than those in the control group ( P 〈0.01). Conclusion Periodontal tissue regeneration combined with orthodontic treatment can significantly reduce inflammatory reactions and improve periodontal conditions in patients with moderate and severe periodontitis.
