走班制学校一起手足口病暴发疫情流行病学调查分析 被引量:1

Study on an outbreak of hand, foot, and mouth disease in a mobile-learning-system school

摘要 目的分析北京市海淀区一起手足口病暴发疫情的流行特征及流行因素。方法采用描述流行病学方法,分析该次暴发疫情中手足口病例的人群、时间和空间分布情况,同时对病例咽拭子标本进行肠道病毒核酸检测。结果疫情发生时间为2017年11月2日至12月3日,共发病34例,罹患率为0.72%(34/4692)。其中,男学生罹患率为1.22%(27/2215),高于女学生[0.31%(6/1917)](χ2=10.65,P=0.001);艺术楼发病率为4.23%(11/260),高于初中楼[1.55%(18/1160)](χ2=7.62,P=0.006);走班制学生发病率为0.74%(31/4162),高于非走班制学生[0.10%(2/2008)](χ2=10.60,P=0.001);参加游学学生发病率为1.98%(28/1416),高于未参加者[0.18%(5/2716)](χ2=37.78,P〈0.01)。对采集的8份病例标本进行肠道病毒核酸检测,结果显示,3份为柯萨奇病毒A6型阳性,1份为肠道病毒通用阳性。结论此疫情是由柯萨奇病毒A6型引起的手足口病暴发疫情,提示走班制教学和游学均可能增加手足口病暴发疫情的风险。 Objective To investigate the epidemiological features and factors of an outbreak of hand, foot, and mouth disease (HFMD) occurred in Haidian District, Beijing. Methods Descriptive analysis was used to analyze of population, time and spatial distribution of HFMD in the outbreak. The enterovirus nucleic acid in throat swab specimens were tested at the same time. Results The epidemic occurred from November 2 to December 3, 2017, with a total of 34 cases. The attack rate was 0.72% (34/4 692). The attack rate of male students was 1.22% (27/2 215), which was higher than that of female students (0.31% (6/1 917)), (χ2= 10.65, P=0.001). The incidence rate in Art Building was 4.23% (11/260), which was higher than that in junior high school buildings (1.55% (18/1 160)) (χ2=7.62, P=0.006). The incidence rate of mobile-learning-system students was 0.74% (31/4 162), which was higher than that of non-mobile-learning-system students (0.10% (2/2 008)) (χ2=1 0.60, P=0.0 1). The incidence rate of participating students was 1.98% (28/1416), which was higher than that of non-participants (0.18% (5/2 716)) (χ237.78, P〈0.01). Enterovirus nucleic acid tests were conducted for 8 specimens collected from cases. The results showed that three samples were positive for Cox A6 and one was positive for enterovirus genus. Conclusions This outbreak was caused by enterovirus Cox A6. It is suggested that all of mobile-learning-system teaching and traveling study may increase the risk of HFMD outbreak.
