摘要
目的:探讨RHD基因外显子多态性与RhD阴性女性妊娠免疫产生抗-D的相关性。方法:收集RhD阴性产妇产后血样148份,使用谱细胞鉴定抗D抗体,PCR-SSP检测RHD基因的10个外显子。结果:148例样本中抗-D阳性105例,抗-D阴性43例;在RHD基因外显子全部缺失的52例(35.14%)样本中,抗-D阳性39例(75.00%)、抗-D阴性13例(25.00%);在RHD基因外显子部分缺失的78例(52.70%)样本中,抗-D阳性56例(71.79%)、抗-D阴性22例(28.21%);在RHD基因外显子无缺失的18例(12.16%)样本中,抗-D阳性10例(55.56%)、抗-D阴性8例(44.44%)。RHD基因外显子全缺失、部分缺失个体免疫产生抗-D与未产生抗-D差异有统计学意义(P<0.05),而RHD基因外显子无缺失个体免疫产生抗-D与未产生抗-D则差异无统计学意义(P>0.05)。结论:妊娠免疫产生抗D抗体与RHD基因外显子不完整性(全缺失、部分缺失)呈正相关。
Objective:To explore the correlation between the polymorphism of RHD gene exons and anti-D caused by pregnancy in RhD-negative women.Method:148 blood samples were collected from RhD-negative parturient after delivery,anti-D was identified by panel cell,the 10 exons of RHD were detected by PCR-SSP.Result:In all the 148 samples,105 were anti-D positive and 43 were anti-D negative;Among 52(35.14%)samples missing all of the RHD exons,39(75.00%)were anti-D positive and 13(25.00%)were anti-D negative;Among 78(52.70%)samples missing the RHD exons partly,56(71.79%)were anti-D positive and 22(28.21%)were anti-D negative;Among 18(12.16%)samples carrying all of the RHD exons,10(55.56%)were anti-D positive and 8(44.44%)were anti-D negative;The difference of anti-D caused by immune or not in the individual missing all or part exons of RHD was significant(P〈0.05),while the difference was not significant in the individual carrying all of the exons of RHD(P〉0.05).Conclusion:The anti-D caused by pregnancy was positively related to the imperfection of exons in RHD gene(totally missing,partly missing).
