摘要
目的探讨综合性评估及个体化干预对糖尿病慢性伤口感染患者自我管理能力及伤口愈合的影响。方法将88例2型糖尿病合并慢性伤口感染患者随机分为二组,观察组和对照组各44例。对照组接受常规治疗及护理措施;观察组在此基础上给予综合性评估与个体化干预,比较二组患者自我管理能力评分及伤口愈合情况。结果观察组自我管理能力各维度评分均高于对照组,饮食行为(t=7.301,P<0.05),运动行为(t=5.394,P<0.05),用药行为(t=4.453,P<0.05),足部护理(t=6.654,P<0.05),血糖监测(t=3.459,P<0.05),比较差异具有统计学意义;观察组伤口面积小于对照组(t=-3.410,P<0.05),压疮愈合计分量表(PUSH)评分低于对照组(t=-2.210,P<0.05),比较差异具有统计学意义。结论对糖尿病合并慢性伤口感染患者实施综合性评估与个体化干预,可提高患者自我管理能力,促进伤口愈合。
Objective To explore the effects of comprehensive evaluation and individualized intervention on self-management and wound healing of patients with diabetes mellitus combined with chronic wound.Methods Totally 88 patients with typeⅡ diabetes mellitus with chronic wounds were randomly divided into two groups：the intervention group and the control group,44 patients in each group.The control group received routine treatment and nursing measures,while the intervention group was given comprehensive evaluation and individualized intervention based on the control.The self-management ability score and wound healing were compared between the two groups.Results Self-management ability score on each dimension in the intervention group was higher than that in the control group.The difference was statistically significant on dietary（t=7.301,P〈0.05）,exercise（t=5.394,P〈0.05）,drug taking（t=4.453,P〈0.05）,foot care（t=6.654,P〈0.05）,blood glucose monitoring（t=3.459,P〈0.05）between the two groups.The wound area of the intervention group was smaller than that of the control group（t=-3.410,P〈0.05）,PUSH score was lower in the intervention group than in the control group（t=-2.210,P〈0.05）.Conclusion The comprehensive evaluation and the individualized intervention,for diabetes mellitus patients with chronic wound infection,can improve the patient′s self-management ability and promote wound healing.
