摘要
目的探讨记忆合金髌骨爪内固定治疗复杂性粉碎性髌骨骨折的临床效果及对患者生活质量的影响。方法采用记忆合金髌骨爪内固定术对62例复杂性粉碎性髌骨骨折患者进行复位固定治疗,术后严格执行康复锻炼。记录患者手术时间、术中出血量、隐性出血量、术后骨愈合时间、末次随访时膝关节恢复优良率及不良事件发生情况。对比患者术前、出院前1 d及术后6个月生活质量评分。结果患者年龄≥60岁和<60岁患者术后骨愈合时间比较,差异有统计学意义(P <0. 05)。末次随访时患者膝关节功能恢复总优良率达93. 55%,不良事件发生率为9. 68%。术后随着恢复时间的延长,患者在生理、心理、社会生活3个维度评分及生活质量总评分呈显著提高趋势,差异均有统计学意义(P <0. 05)。结论记忆合金髌骨爪治疗复杂性粉碎性髌骨骨折疗效确切,具有操作简单、固定可靠、骨愈合良好、并发症少等优点。
Objective To investigate effect of internal fixation of memory alloy patella claw on curative efficacy and patients′ quality of life for the treatment of complicated comminuted patella fracture. Methods Memory alloy tibial internal fixation was used to treat 62 patients with complex comminuted tibial fractures, and strict rehabilitation exercises after surgery was performed. The operative time, intraoperative blood loss, occult blood loss, postoperative bone healing time, good recovery of knee joints at the final follow-up, and adverse events were recorded. The quality of life scores were compared before surgery, 1 day before discharge, and 6 months after surgery. Results There was a statistically significant difference in bone healing time between patients aged ≥ 60 and〈60 years old ( P 〈0.05). At the last follow-up, the total excellent and good rate of knee joint function recovery was 93.55%, and the adverse event rate was 9.68%. With the prolonging of recovery time, the scores of three dimensions such as physical, mental and social life and the total quality of life scores of patients had significant increase, and showed a statistically significant differences ( P 〈0.05). Conclusion The internal fixation of memory alloy patella claw is effective in the treatment of complex comminuted tibial fractures. It has the advantages of simple operation, reliable fixation, good bone healing and less complications.
