摘要
目的探讨炎性介质血清淀粉样蛋白A(SAA)、白细胞介素6(IL-6)和人附睾蛋白4(HE4)在卵巢癌患者中的表达,并分析三者的相关性及在卵巢癌早期诊断中的价值。方法回顾性分析唐山市妇幼保健院2014-07~2017-12初诊入院的卵巢肿瘤患者120例,其中恶性肿瘤48例(卵巢癌组),良性肿瘤72例(卵巢良性肿瘤组),另外选取健康对照组50例。采用化学发光法检测受检者HE4的含量;采用酶联吸附实验(ELISA)测定SAA和IL-6的含量;同时应用ROC曲线分析三者对卵巢癌的诊断价值。结果卵巢癌患者全血SAA、IL-6、HE4明显高于卵巢良性肿瘤组和正常对照组。卵巢癌组SAA与HE4、IL-6正相关(r_(HE4)=0. 67,P <0. 05; r_(IL-6)=0. 54,P <0. 05)。三者均随临床分期的升高呈增高趋势,与临床分期正相关;SAA、IL-6、HE4的表达随着卵巢癌分化程度的降低而增高,三者与病理分化程度呈负相关。ROC曲线分析显示,SAA、IL-6、HE4三者联合检测诊断卵巢癌的ROC曲线下的面积0. 987。三者联合诊断效率高于两两联合检测。结论 SAA与IL-6、HE4检测可以很好地鉴别卵巢良恶性肿瘤,具有较高的诊断价值,可作为卵巢癌临床分期和病理分化程度的重要指标。
Objective To investigate the expression of inflammatory mediators sermn amyloid A protein( SAA), interleukin-6 (IL-6) and human epididymis protein 4 (HFA) in ovarian cancer patients and to analyze their correlation and role in early diagnosis of ovarian cancer. Methods Clinal data of ovarian tumor patients were retrospectively analyzed in Tangshan Women and Children' s Health Hospital from July 2014 to December 2017. Totally 120 patients(48 malignant tumors and 72 benign tumors) and 50 healthy controls were selected. The level of HE4 in the subjects was detected by ehemilumineseenee, the contents of SAA and IL-6 were measured by enzyme-linked adsorption assay-(ELISA) , and the diagnostic value of ultrasonography in ovarian cancer was analyzed by ROC curve. Results The levels of SAA, IL-6 and HE4 were significantly higher in patients with ovarian cancer than those in ovarian benign tumors and normal controls. The level of SAA was positively correlated with IL-6 ( r = 0.67 ,P 〈 0.05 ) and HE4 ( r = 0.54 ,P 〈 0.05 ) in ovarian cancer patients. The expression levels of SAA, IL-6 and HE4 were positively correlated with clinical stage, but negatively correlated with pathological grade. ROC curve analysis showed that the area under the ROC curve for the combined detection of SAA, IL-6, and HE4 in the diagnosis of ovarian cancer was 0. 987. The three combined diagnostic efficiency was higher than that of two combination. Conclusion The combined detection of SAA, IL-6 and HE4 can be used to distinguish benign from malignant ovarian tumors. They can be used as important indicators of ovarian cancer in clinical staging and pathological grade.
