不同甲状腺功能状态下血清FT3、FT4和hTSH的临床检测意义 被引量:4

摘要 目的探讨血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)及超灵敏促甲状腺素(hTSH)对不同甲状腺疾病的临床检测意义。方法回顾性分析2017年1月至2018年1月甲状腺疾病患者的临床资料,将118例甲状腺功能亢进(甲亢)患者设为观察1组,将118例甲状腺功能减退(甲减)患者设为观察2组,另选118例健康体检人群设为对照组,检测三组人群的血清FT3、FT4及hTSH水平,探讨其对甲状腺疾病的诊断价值。结果 (1)观察1组、观察2组的血清FT3、FT4及hTSH水平明显异常于对照组,且观察1组的血清FT3、FT4水平明显高于观察2组,hTSH水平明显低于观察2组(P <0. 05);(2)经测定,血清hTSH诊断甲亢、甲减的符合率(92. 37%、98. 31%)明显高于血清FT3(76. 27%、72. 88%)、FT4(75. 42%、70. 34%)(P <0. 05),但血清FT3、FT4诊断甲亢、甲减的符合率比较,差异未见统计学意义(P> 0. 05)。结论血清FT3、FT4及hTSH水平与甲状腺疾病关系密切,尤其是hTSH,可作为甲状腺疾病鉴别诊断、病情评估及指导用药的敏感指标。 Objective To investigate the clinical significance of serum free triiodothyronine( FT3),free thyroxine( FT4) and hypersensitive thyrotropin( hTSH) in detection of different thyroid diseases. Methods One hundred and eighty cases with hyperthyroidism and who treated in our hospital from January,2017 to January,2018 were selected as observation group 1,and 118 cases of patients with hypothyroidism( hypothyroidism) were selected as the observation group 2. And other 118 cases of healthy physical examination was selected as the control group. Then,the serum levels of FT3,FT4 and hTSH of three groups of people were detected,and their diagnostic value for thyroid diseases were explored. Results(1)The levels of serum FT3,FT4 and hTSH in the observation group 1 and observation group 2 were significantly higher in the control group than in the control group. And the serum levels of FT3 and FT4 in the observation group 1 was significantly higher than those in the observation group 2,and the level of hTSH was significantly lower than that in the observation group 2( P〈0. 05).(2) By the measurement,the coincidence rate of serum hTSH in diagnosis of hyperthyreosis and hypothyroidism was92. 37%,98. 31%,which was significantly higher than that of serum FT3( 76. 27%,72. 88%) and FT4( 75. 42%,70. 34%)( P〈0. 05). But the coincidence rate of serum FT3 and FT4 in diagnosis of hyperthyreosis and hypothyroidism was not statistically significant( P〉0. 05). Conclusions The level of serum FT3,FT4 and hTSH is closely related to thyroid disease,especially hTSH,which can be used as a sensitive index for differential diagnosis,evaluation and guidance of thyroid disease.
