摘要
目的探讨肿瘤标志物联检在肺癌诊断中的应用价值以及在肺癌中医辨证分型诊断中的应用价值。方法选取2017年1月—2018年1月收治的85例肺癌患者纳入肺癌组,同时选取同期到本院就诊的良性病变患者62例纳入良性病变组,最后再选取同期到院进行健康检查的健康体检者70例纳为正常对照组;所有纳入研究的对象均对胚抗原(CEA)、神经元烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)以及鳞状细胞癌抗原(SCC-Ag)等肿瘤标志物进行联合检测;观察各肿瘤标志物在肺癌诊断以及肺癌与中医辨证分型中的应用价值。结果肺癌组患者CEA、NSE、CYFRA21-1以及SCC-Ag等平均水平均分别高于良性病变组与正常对照组(P <0. 05);相对于肿瘤标志物单独检测,联检的灵敏度与诊断符合率更高(P <0. 05);肺癌不同中医辨证类型其肿瘤标志物水平比较差异具有统计学意义(P <0. 05)。结论肿瘤标志物联检一方面能够有助于临床对肺癌的诊断,另一方面,能够为肺癌中医辨证分型提供参考依据,从而为临床进一步诊治提供有效的指导依据。
Objective To explore the application value of combined detection of tumor markers in the diagnosis of lung cancer and the diagnosis of TCM syndrome differentiation of lung cancer. Methods Eighty-five patients with lung cancer admitted in our hospital from January 2017 to January 2018 were included in the lung cancer group,while 62 patients with benign lesions who were admitted to our hospital at the same time were selected to be included in the benign lesion group. At last,70 healthy persons who came to hospital for health examination at the same time were selected as normal control group. All the subjects were given embryo antigen CEA,neuronal enolase NSE,cytokeratin 19 fragments CYFRA21. The application value of tumor markers in the diagnosis of lung cancer and the differentiation between lung cancer and TCM syndromes by combined detection of tumor markers such as squamous cell carcinoma antigen（SCC-Agg） and squamous cell carcinoma antigen（SCC-Agg） were observed. Results The average levels of Cyfra 21-1 and SCC-Ag in lung cancer group were higher than those in benign lesion group and normal control group（P〈0. 05）. The sensitivity of combined therapy was higher than that of diagnosis（P〈0. 05）,and the level of tumor markers was significantly different in different TCM differentiation types of lung cancer（P〈0. 05）.Conclusion On the one hand,the combined detection of tumor markers can be helpful to the diagnosis of lung cancer. On the other hand,it can provide a reference for the differentiation of symptoms and signs of lung cancer,thus providing an effective basis for the further diagnosis and treatment of lung cancer.
