摘要
目的探讨分化型甲状腺癌患者的钠/碘转运蛋白(NIS)表达情况及其与甲状腺超声特征的关系。方法回顾性分析45例甲状腺结节患者(甲状腺结节组)、51例甲状腺乳头状癌(PTC)患者(PTC组)和21例甲状腺滤泡状癌(FTC)患者(FTC组)的临床资料,所有患者均经彩色多普勒超声检测。采用免疫组织化学法检测3组患者的NIS表达水平。分析PTC组和FTC组患者NIS表达情况与甲状腺超声特征的关系。结果 PTC组和FTC组患者的NIS阳性表达率和免疫组化评分(IHS)均高于甲状腺结节组,差异均有统计学意义(P﹤0.05)。PTC组患者的IHS高于FTC组,差异有统计学意义(P﹤0.05);而两组患者的NIS阳性表达率比较,差异无统计学意义(P﹥0.05)。甲状腺超声结果显示,PTC组和FTC组患者的颈部淋巴结肿大情况比较,差异无统计学意义(P﹥0.05);两组患者在肿瘤直径、回声、形态、边界、钙化以及血流方面比较,差异均有统计学意义(P﹤0.05)。PTC组中,NIS阳性表达和阴性表达患者在回声、形态、边界、钙化以及颈部淋巴结肿大方面比较,差异均无统计学意义(P﹥0.05);而在肿瘤直径和血流方面比较,差异均有统计学意义(P﹤0.05),NIS阳性表达患者血流表现以点条状为主,NIS阴性表达患者则以血流丰富为主。FTC组中,NIS阳性表达和阴性表达患者的上述甲状腺超声特征比较,差异均无统计学意义(P﹥0.05)。结论彩色多普勒超声可以有效地鉴别PTC和FTC。
Objective To investigate the expression of sodium/iodide symporter (NIS) in differentiated thyroid carcinoma and its relationship wifll the characteristics of thyroid ultrasound.Method The clinical data of 45 patients with thyrind nodule (thyroid nodule group), 51 patients with papillary thyroid carcinoma (PTC group) and 21 patients with tollicular thyroid carcinoma (FTC group) were retrospectively analyzed. All patients were detected by color Doppler ultrasound. The immunohistochemical method was used to detect the expressions of NIS in the three groups. Then the relationship of expressions of NIS in PTC and FTC group and thyroid ultrasound timings were analyzed. Result The positive expression rates of NIS and immunohistochemical scores (IHS) in PTC and FTC group were significantly higher than those of the thyroid nodule group (P〈0.05). The IHS in the PTC group was significantly higher than that in the FTC group (P〈 0.05), while there was no significant difference in the positive expression rates of NIS between the two groups (P〉0.05). Thyroid ultrasonography showed that there was no significant difference in cervical lymph nodes between PTC group and FTC group (P〉0.05); while the differences of diameter, echo, shape, boundary, calcification and blood flow between two groups were all statistically significant (P〈0.05). In PTC group, there was no significant difference in echo, shape, bound- ary, calcification and cervical lymph nodes between positive and negative NIS expression group (P〉0.05); but there were significant differences in tumor diameter and blood flow between the two groups (P〈0.05), and the blood flow in patients with positive NIS expression mainly performed as point bars, while the blood flow in patients with negative NIS expres- sion mainly performed as rich blood flow. In the FTC group, there was no significant difference in thyroid ultrasonography between positive and negative NIS expression group (P〉0.05). Conclusion Color Doppler ultrasound can effectively identify PTC and FTC.
