摘要
目的:检测葡萄糖转运蛋白-1(GLUT-1)和血管内皮生长因子A(VEGF-A)在结直肠癌组织中的表达,评价其与结直肠癌临床病理特征和预后的关系。方法:采用免疫组织化学法检测95例结直肠癌组织及癌旁正常组织中的GLUT-1和VEGF-A的表达,分析两者表达相关性及与临床病理特征和预后的关系。结果:GLUT-1和VEGF-A在癌组织中表达明显高于癌旁组织(P<0.05);GLUT-1和VEGF-A的表达在有无淋巴结转移、不同TNM分期间差异有统计学意义(P<0.05),在不同年龄、性别、肿瘤大小和分化程度间差异均无统计学意义(P>0.05);Spearman相关性分析显示GLUT-1、VEGF-A两者在结直肠癌组化中的表达呈正相关(r=0.510,P<0.05);Kaplan-Meier生存分析发现,GLUT-1和VEGF-A均为阳性表达患者的生存预后明显差于其他患者(χ~2=6.79,P=0.009);单因素分析显示肿瘤淋巴结转移情况、TNM分期和GLUT-1、VEGF-A表达是否均为阳性是影响患者生存的危险因素(P<0.05),多因素分析显示,肿瘤淋巴结转移是影响患者生存的独立危险因素(P<0.05)。结论:结直肠癌组织中GLUT-1和VEGF-A的表达高于正常组织,两者共同参与了结直肠癌的发生发展和转移过程,对患者的治疗和预后评估具有一定的参考价值。
Objective: To detect the expression of glucose transporter-1(GLUT-1) and vascular endothelial growth factor A(VEGF-A) in colorectal cancer and evaluate its relationship with clinicopathological features and prognosis of colorectal cancer. Methods: Immunohistochemistry was used to detect GLUT-1 and VEGF-A in 95 cases of colorectal cancer tissues and adjacent normal tissues. Their relationship with clinicopathological features and prognosis was analyzed. Results: The expression of GLUT-1 and VEGF-A in colorectal cancer tissue was significantly higher than that in paracancerous tissues(P〈0.05). The expression of GLUT-1 and VEGF-A was significantly different from that of lymph node metastasis and TNM stage, there was no statistically significant difference in age, gender, tumor size and differentiation(P〉0.05). Spearman correlation analysis showed a positive correlation between GLUT-1 and VEGF-A expression(r=0.510,P〈0.05). GLUT-1 and VEGF- A were analyzed by Kaplan-Meier survival analysis and the prognosis of patients with both of positive expression of GLUT-1 and VEGF-A was significantly lower than that of other patients(χ2=6.79, P=0.009). Univariate analysis showed that lymph node metastasis, TNM staging, and both of GLUT-1 and VEGF-A expression were all positive risk factors for survival. Multivariate analysis showed that only the tumor lymph node metastasis was a risk factor that affects the patient's survival. Conclusion: GLUT-1 and VEGF-A are higher expressed in colorectal cancer tissues than adjacent normal tissues. They are involved in the occurrence, development and metastasis of colorectal cancer, and have a certain reference value for the treatment and prognosis of patients.
