摘要
目的观察对于新生儿缺氧缺血性脑病发生脑水肿时应用高渗钠的临床应用分析。方法将我院2016年1月~2018年2月收治以新生儿缺氧缺血性脑病发生脑水肿为诊断的患儿80例,依照患儿就诊顺序分成两组,每组40例。两组均给予对症治疗,其中对照组适量给予甘露醇注射液配合限制水摄入治疗,高渗组给予高渗钠即3%氯化钠纠正治疗。将两组患儿治疗前、后3d、5d、7d、1个月新生儿行为测定(NBNA评分),及治疗前、后血液中钠离子水平、及脑水肿消失时间,住院天数对照。结果治疗后高渗组钠离子水平明显高于对照组,且高渗组脑水肿消失时间及住院时间明显少于对照组,高渗组治疗后3、5、7d评分明显高于对照组,差异有统计学意义(P <0.01),治疗后1个月随诊,两组评分比较差异无统计学意义(P>0.05)。结论对于新生儿缺氧缺血性脑病患儿而言,采取高渗钠治疗,可有效提高NBNA评分,改善血液中钠离子水平,缩短脑水肿消失时间及住院时间,减轻患儿痛苦及家庭负担,效果理想。
Objective Clinical application of hypertonic sodium in neonatal hypoxic-ischemic encephalopathy with brain edema. Methods From January 2016 to February 2018,80 eases of neonatal hypoxic-ischemic encephalopathy with brain edema were be treate in our hospital recently,they were divided into two groups according to the order of visiting children,40 eases in each group.Symptomatic treatment was given to both groups,the control group was given appropriate amount of mannitol injection combined with limited water intake treatment,hypertonic group was given hypertonic sodium,that is 3% sodium chloride corrective treatment.The neonatal behavior (NBNA score) was measured before,after 3,5,7 and 1 month treatment,and the levels of sodium ion in blood and the disappearance time of brain edema before and after treatment,as well as the days of hospitalization were compared between the two groups. Results After treatment,the level of sodium ion in hypertonie group was significantly higher than that in control group,the disappearance time and hospitalization time of brain edema in hyperosmotie group were significantly shorter than those in control group.The scores of 3,5 and 7 days after treatment in hyperosmotie group were significantly higher than those in control group,the difference was statistically significant (P 〈 0.05),after 1 month treatment of score in two groups,there was no significant difference(P 〉 0.05). Conclusion For neonates with hypoxie-isehemie eneephalopathy,hypertonie sodium therapy can effectively improve NBNA score,improve the level of sodium ions in blood,shorten the disappearance time of brain edema and hospitalization time,alleviate the pain and family burden of the child, the effect is ideal.
