摘要
目的了解北京市海淀医院2017年分离细菌对抗菌药物的耐药性。方法收集非重复分离菌株,自动化仪器法进行药敏试验,按CLSI 2016年版判读药敏结果,采用WHONET 5. 6软件进行分析。结果 2 166株细菌中,革兰阴性菌占71. 4%,革兰阳性菌占28. 6%。最常见的细菌为大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌。耐甲氧西林金黄色葡萄球菌和凝固酶阴性葡萄球菌的检出率分别为39. 1%和73. 2%;未发现对万古霉素耐药的葡萄球菌。粪肠球菌对大多数抗菌药物的耐药率低于屎肠球菌;发现万古霉素耐药肠球菌。克雷伯菌属对亚胺培南的耐药率为16. 1%,其他肠杆菌科细菌对碳青霉烯类耐药率≤10. 7%。鲍曼不动杆菌和铜绿假单胞菌对亚胺培南的耐药率分别为33. 2%和22. 6%。多重耐药鲍曼不动杆菌和铜绿假单胞菌的检出率分别为34. 2%和20. 3%。结论细菌对抗菌药物的耐药率呈下降趋势,应持续采取有效措施降低多重耐药菌的比例。
Objective To investigate the antimicrobial resistance profile in the clinical bacterial strains isolated from Haidian Hospital in 2017. Methods Non-repetitive isolates were collected,and the drug sensitivity test was performed by automated instrumentation. The drug susceptibility results were interpreted according to the CLSI 2016 edition and analyzed using WHONET 5. 6 software. Results Of the 2 166 isolates,71. 4% were gram-negative,and 28. 6% were gram-positive. The most frequently isolated bacteria were E. coli,K. pneumoniae,P. aeruginosa. Methicillin resistant strains in S. aureus and coagulase negative Staphylococcus accounted for 39. 1% and 73. 2%,respectively. No Staphylococcal strains were resistant to vancomycin. The resistance rate of E. faecalis strains to most of the drugs was much lower than those of E. faecium. Several strains of E. faecium were found resistant to vancomycin. Klebsiella spp had a high drug resistance rate of 16. 1% to imipenem.The resistance rate of other Enterobacteriaceae bacteria to carbapenems was ≤10. 7%. And 33. 2% of A. baumannii isolates were resistant to imipenem. The resistance rates of Acinetobacter baumannii and Pseudomonas aeruginosa to imipenem were33. 2% and 22. 6%,respectively. The detection rates of multidrug-resistant Acinetobacter baumannii and Pseudomonas aeruginosa were 34. 2% and 20. 3%,respectively. Conclusion Bacterial resistance is decreasing,but it is necessary to continue to take effective measures to reduce multidrug-resistant bacteria.
