摘要
目的分析上海市浦东新区8~10岁学龄儿童尿碘水平与甲状腺疾病的关系。方法通过整群随机的方法抽取上海市浦东新区8~10岁学龄儿童340例,进行甲状腺B超检查,并采集儿童尿液进行尿碘检测,观察分析检测结果。结果 340例学龄儿童中,24例儿童发生甲状腺疾病,发生率为7.06%(24/340);不同性别、不同年龄段的甲状腺疾病发生率、尿碘中位数比较,差异均无统计学意义(P>0.05)。尿碘水平≥300μg/L学龄儿童的甲状腺疾病发生率为5.26%,尿碘水平为100μg/L组的甲状腺疾病发生率为6.84%,尿碘水平为200μg/L组学龄儿童的甲状腺疾病发生率为8.97%,尿碘水平为0μg/L组学龄儿童的甲状腺疾病发生率为7.25%。各组甲状腺疾病发生率比较,差异均无统计学意义(P>0.05)。有结节组、无结节组学龄儿童的尿碘中位数分布为192.66和179.48μg/L,两者比较,差异无统计学意义(P>0.05)。结论上海市浦东新区8~10岁学龄儿童的碘营养状况比较理想,当碘营养状况处在WHO所提出的合理范围内时,发生甲状腺疾病的几率较低。
Objective To analyze the relationship between urinary iodine levels and thyroid diseases in school children aged 8-10 in Shanghai Pudong New area. Methods 340 school children aged 8-10 in Shanghai Pudong New area were selected by cluster random sampling method, all the children were given thyroid B ultrasonic examination. Urine was collected from the children for urinary iodine detection, and the results were analyzed. Results Among the 340 school children, 24 children with thyroid diseases were found, the prevalence of thyroid diseases was 7.06%(24/340);and there was no significant difference in the prevalence of thyroid disease by sex and age as well asurinary iodine median(P〉0.05).The school children with urinary iodine level≥300μg/L had prevalence of thyroid disease(5.26%); urinary iodine level of 100μg/L, the prevalence of thyroid diseases was 6.84%. The prevalence of thyroid diseases in the urinary iodine level of 200μg/L-group was 8.97%,urinary iodine level of 0μg/L, the prevalence of thyroid diseases was 7.25%. The difference of the prevalence of thyroid diseases was not statistically significant in each groups(P〉0.05). The median distribution of urinary iodine in the nodular group and the no nodular group was 192.66μg/L,179.48μg/L, there was no significant difference of the two groups(P〉0.05).Conclusions The iodine nutritional status of school children aged 8 to 10 in Shanghai Pudong New area is ideal, and the prevalence of thyroid diseases is lower when iodine nutrition status is within the reasonable range recommended by WHO.
