海马硬化所致颞叶癫痫SPECT与MRI脑血流灌注对照研究 被引量:8

摘要 目的通过应用单光子发射断层扫描(single-photon emission tomography,SPECT)脑血流灌注显像及动脉自旋标记(arterial spin labeling,ASL)脑血流灌注成像技术对海马硬化(hippocampal sclerosis,HS)所致颞叶癫痫(intractabletemporal lobe epilepsy,TLE)患者脑血流灌注,特别是海马区血流灌注进行定性及半定量测量,并比较其相关性。方法搜集经手术及病理证实为HS的18例TLE患者。选择性别、年龄匹配的21例健康志愿者作为对照组。18例患者及3例健康志愿者行SPECT脑血流灌注显像,全部患者及志愿者行ASL脑血流灌注成像。对HS所致TLE患者脑内,特别是海马区的血流灌注进行定性及定量分析,分别比较应用两种血流灌注方法(SPECT、ASL)测得的HS患者病变侧、对侧及与对照组海马区rCBF的差异。结果应用SPECT及ASL两种脑血流灌注显像技术测得HS所致TLE患者病变侧海马区rCBF均较对侧减低,病变侧与对侧海马区rCBF有显著性差异(P <0. 01);病变侧海马区rCBF较对照组明显减低,病变侧与对照组海马区rCBF有非常显著性差异(P <0. 01);对侧与对照组海马区rCBF有无显著性差异(P> 0. 05)。结论SPECT脑血流灌注显像和ASL脑血流灌注成像技术定性分析HS所致TLE脑血流灌注改变一致性较好,均证实HS所致TLE病变侧海马区血流灌注减低。 Objective Through the application of single photon emission tomograph (SPECT) and arterial spin labeling (ASL) technology to hippocampal sclerosis temporal lobe epilepsy patients cerebral perfusion, especially the hippocampus blood perfusion for qualitative and semi-quantitative measurement, to compare the change in cortices blood perfusion and the hippocampus blood perfusion in the hippocampal sclerosis (HS) temporal lobe epilepsy (TLE) patients. Methods Eighteen patients were diagnosed hippocampal sclerosis by surgery and pathology. Twenty-one healthy volunteers were chosen as control groups, with sex and age-matched paients. Eighteen patients and three volunteers underwent SPECT 99m Tc - ECD cerebral blood perfusion imaging and magnetic resonance imaging. All patients and volunteers underwent the 3.0 T super-conductive MR scanner and ASL. Bilateral hippocampus regiongal cerebral blood flow (rCBF) were measured with the region of interest (ROI) technology in the SPECT and ASL images. The conformity between SPECT and ASL cerebral blood perfusion imagings was compared. Results Both SPECT 99m Tc-ECD cerebral blood perfusion imaging and ASL cerebral blood perfusion imaging demonstrated that ipsilateral hippocampus rCBF was decreased compared with contralateral hippocampus rCBF in patients with intractable TLE ( P 〈0.01), and ipsilateral hippocampus rCBF was significantly decreased compared with control participants ( P 〈0.01). There were statistically significant differences in rCBF between the contralateral hippocampus and control participants ( P 〈0.05). Conclusion SPECT and ASL cerebral blood perfusion imagings have good conformity. There are abnormal rCBF decreases in the ipsilateral hippocampus of HS TLE patients. They are confirmed by ASL and SPECT cerebral blood perfusion imaging.
