摘要
目的了解双源CT门静脉成像(CTPV)技术在观察肝硬化门静脉侧支循环的临床应用价值。方法选择2017年3月至2018年3月来我院诊断治疗80例肝硬化患者为研究组,并选择同期来我院接受正常体检的60例健康体检的参试者为对照组,两组分别进行上腹部CT增强检查并实施门静脉成像,研究组并做了胃镜检查,分别测量两组参试者门静脉主干(MPV)、脾静脉(SPV)、肝内门静脉左支(IHLPV)、肝内门静脉右支(IHRPV)和胃左静脉(LGV)直径及食管下段静脉,并将各统计结果与Child-Pugh分级进行对比分析及统计学处理。结果 CT门静脉成像技术能清楚的显示肝硬化门静脉高压患者的侧支循环血管情况,两组参试者门静脉分支及属支直径差异明显,有统计学差异(P<0.05),研究组侧支循环血管直径均数对比,不同肝功能分级存在明显差异,差异有统计学意义(P<0.05)。CTPV与胃镜两种检查方法对于食管下段静脉曲张检出比差别不大,Kappa大于0.75。结论双源CT门静脉成像可清晰显示肝硬化门静脉及其侧支循环情况,对于判断肝硬化患者肝功能受损程度有较高的价值。
Objective To investigate the clinical value of dual-source CT portography(CTPV)in observing collateral circulation of portal vein in hepatocirrhosis.Methods 80 patients with hepatocirrhosis diagnosed and treated in our hospital from March 2017 to March 2018 were selected as the study group,and 60 healthy people who received normal physical examination in our hospital during the same period were selected as the control group.The two groups underwent enhanced CT examination of upper abdomen and portal vein imaging,and the study group underwent gastroscopy.The main portal vein(MPV)and splenic vein of the two groups were measured respectively.(SPV),left branch of intrahepatic portal vein(IHLPV),right branch of intrahepatic portal vein(IHRPV)and left gastric vein(LGV)diameters and lower esophageal veins were compared and analyzed with Child-Pugh classification.Results CT portography can clearly show the collateral vessels in patients with hepatocirrhosis and portal hypertension.The diameter of portal vein branches and subordinate branches in the two groups was significantly different(P<0.05).The mean diameter of collateral vessels in the study group was compared,and there were significant differences in different liver function grades(P<0.05).CTPV and gastroscopy showed no significant difference in the detection ratio of lower esophageal varices,with Kappa greater than 0.75.Conclusion Dual-source CT portography can clearly show the portal vein and its collateral circulation in hepatocirrhosis patients,and it has a high value in judging the degree of liver function damage in hepatocirrhosis patients.
