摘要
目的 :探讨窒息新生儿脐血生化指标变化的临床意义。方法 :胎儿娩出后 ,在行 Apgar评分及复苏的同时 ,在脐带距胎儿脐根部 5~ 10 cm处沿脐动脉向胎儿端进针 ,取静脉血 2 ml,断脐后再取股静脉血 2 ml,进行血钾、钠、氯、钙、镁、糖、尿素氮测定 ,并根据窒息程度分组比较。结果 :窒息组脐血钠、氯、钙、镁明显低于对照组 ,而血糖、尿素氮均高于对照组 ,有极显著性差异 (P均 <0 .0 1)。股静脉血检查结果与脐血结果比较 ,血钾、钠、氯、钙、镁、血糖、尿素氮值均无显著差异 (P均 >0 .0 5 )。结论 :脐血生化指标监测可反映窒息程度 ,并能替代股静脉血检查 ,有利于早期判断病情 ,指导临床用药。
Objective:To investigate the changes of biochemistry indexes of infants' umbilical cord and its clinical significance.Methods:After embryo was birth Apgar scale was evaluated and resuscitation was performed,simultaneously puncture was made in 5-10 cm apart from the foot of umbilical cord and 2 ml venous blood were taken.After umbilial cord was cut off,2 ml blood of femoral venous were taken and the biochemstry indexes such as blood potassium sodium,chlorine,blood glucose were tested.Infants were divided into two groups according to the level of suffocation and compared to each other.Results:The umbilical cord blood sodium chlorine of the suffocation group was significantly lower than the control group, but blood glucose and blood urea nitrogen was higher,they were remarkable difference (all P<0.01).There was no difference between the blood of femoral venous and umbilical cord.Conclusions:Umbilical cord biochemistry can reflect the level of suffocation and take place of venous blood of femoralus,it has beneficial to judge the patients' condition and guide the clinical treatment.
